- Sliding scale fee for uninsured. - Root canal procedure $700.

- Sliding scale fee for uninsured ($10). - Small fees for lab work and root canals.

- Up to $500 for crowns. - Sliding scale fee for uninsured ranging

from $0-$200.

- Patient pays lab fees.

$25 cash nonrefundable service/records fee per patient per cycle of visits.
