SUBSTITUTE TEACHER APPLICATION PROCESS

Thank you for your interest in substitute teaching for the Lexington R-V School District. To be eligible to substitute you must obtain a substitute certificate from the Department of Elementary and Secondary Education or hold a valid teaching certificate from the State of Missouri. Below are directions to assist you with this process:

1. Set an appointment with 3M/Cogent to capture your fingerprints. {Information sheet included).

2. Apply for substitute certificate on-;-line with DESE at http:Udese.. (Information sheet included).

3. Mail original transcripts to DESE (write in educator ID or SS number and "SUB CERT") DESE, Attn: Certification, P.O. Box 480, Jefferson City, Mo 65102. Certificate will be issued not mailed. Applicant can print a copy on their computer.

4. Complete and return this application packet to Lexington R-V School District. .

GENERAL INFORMATION

The rate of pay for substitute teachers is $90 per day. Substitute teachers are paid the 22nd of each month by direct deposit.A long-term substitute teacher who substitute for the same teacher more than 10 consecutive days will receive higher rate of pay depending on the base salary.

Educator Certification

Help Guide for Online Certification System

All certification requests will be made through the online educator certification system. To access the system, you must first create and register a user name and password at the following web page: . 1. Register a. Click "Register" in the lower portion of the page b. Complete the registration questions c. Click "Create User" (a confirmation message will appear) d. Click "Return to DESE Login page" link NOTE: Do not create more than one username. Creating multiple usernames will cause the system to deny you access to your profile page and certification records. If you have forgotten your username or password, you can click on "Forgot Username/Password?" in the lower section of the login page to request your username or to reset your password. 2. CONNECT USERNAME/PASSWORD TO WEB APPLICATIONS SYSTEM () a. Enter your username and password and click "Login" to access the User Application page. b. Locate the "Office of Educator Quality" heading near the center of the page c. Click "Educator Certification System - Request Educator Access" link d. Click "Submit" e. Click "Close" in the small confirmation window f. Locate the "Office of Educator Quality" heading near the center of the page once again g. Click on "Educator Certification System" link h. Enter your Social Security Number (SSN) and date of birth (DOB) i. Click "Submit" j. Enter or verify your Profile information (proper/legal name - no nicknames) and click "Save Profile" k. Scroll down and complete or edit the Contact Information* section and click "Save" *An e-mail address must be included in this section, as all correspondence from Educator Certification will be sent to you by email. 3. APPLY FOR CERTIFICATION a. Locate the appropriate application (Initial, Substitute, Non-MO Graduate, etc.) under "New Applications" in the menu on the left side of your Profile page.

Mail supporting documents as indicated on application checklist**, if applicable *Complete the application, answering the Professional Conduct questions on the application truthfully and reviewing the Sworn Statement before electronically signing by selecting "I Accept." This agreement provides DESE with permission to verify your professional conduct statements. **Your internet browser must allow "pop-ups" in order to view the application checklist.

Additional Information for Returning Users Accessing your Profile You can find the login page by clicking on "Web Applications" in the bottom section of the DESE Homepage or on "DESE Web Applications" on the DESE Educator Certification main page. ? In your online Profile, you will be assigned an Educator ID number that will appear below your SSN. This is a

unique identification number that you can use in place of your SSN when corresponding with our office.

submit an Update Personal/Education Information Form and supporting documents in order to request changes to any of these fields.

? You may change your address, telephone number(s), and e-mail address by clicking on "Edit Contact

FBI/Highway Patrol Background Check Procedures

Four-Digit Registration Code

A four-digit registration code is required to pre-register. The four-digit registration code ensures that the results of the background check are returned to the correct agency. Registration must be completed for BOTH an FBI and Highway Patrol check. Total cost for fingerprinting is $44.80. Completing only a Highway Patrol open records check is NOT sufficient.

Certified and non-certified staff will need to contact the employing Missouri Public School District for the correct four digit registration code prior to pre-registering for fingerprinting. If employed by more than one school district, choose only one district's registration code.

Educators who are not employed with a Missouri public school district should use registration code 2300. Substitute Teachers who are not employed with a public district should use registration code 2301.

Registration Process - Missouri Residents All individuals must pre-register online for fingerprinting through the State Highway Patrol's Missouri Automated Criminal History Site (MACHS). The registration site is located at machs. Individuals without access to the Internet may contact the fingerprint processing company, 3M/Cogent, directly at 1-877-862-2425 to have a Fingerprint Services Representative conduct this registration on their behalf. A four-digit registration code is required in order to complete the online registration process.

Registration Process - Non-Missouri Residents Residents from outside the state of Missouri who are unable to schedule an appointment for fingerprinting in Missouri may mail fingerprint cards directly to 3M/COGENT. The mandatory information that must be included on the FBI fingerprint cards is located on page 2 of this document. Questions about this process may be directed to 3M/Cogent at 1-877-862-2425 or to the Missouri State Highway Patrol at 573-526-6312.

Fingerprinting Fee The fee for the fingerprinting process for both Missouri residents and Non-Missouri residents is $44.80. The payment may be made online at the time of registration or at the fingerprinting appointment. Reminder: The correct fee for fingerprinting is $44.80.

Fingerprint Results Fingerprint results for educators, substitutes and student teachers will be recorded automatically on individual profile pages in the online Educator Certification System. Results for non-certified staff members and bus drivers will be forwarded to the appropriate school district based upon the registration code provided. Results offingerprints are generally reported to the office of Educator Certification within 2-3 weeks from the date of appointment.

With many changes in health insurance for Americans, some will have new opportunities to enroll for coverage through an electronic insurance exchange or "Marketplace". The marketplace will offer various levels of health plan coverage called "metals", from the least expensive "bronze" level to the richest plans at the "platinum" level. Pricing for the new plans will not be affected by medical conditions and they will cover pre-existing conditions.

Americans w h o do not have coverage offered through their employer that is "adequate!' and "affordable" may be eligible for a subsidy to offset some of the cost of Marketplace plans, depending on their household income. Lexington R-V School District is pleased to offer our benefit eligible employees health coverage that both meets and exceeds the requirements of "adequate" and "affordable". While this means that you will not be eligible for a tax credit subsidy when considering coverage for yourself or the Marketplace plans, this does not disqualify your family members from obtaining a subsidy in the Marketplace.

Our part-time, or non-benefit eligible employees may also qualify for subsidies in the Market place Exchanges.

Most Americans will be required to have health insurance coverage beginning January 1, 2014. Americans t at elect to go without health insurance coverage will- be taxed. The tax penalty is based on a percentage of gross income, and applies to each uninsured household member, including children.

Penalties for not having health coverage (are the greater of): ?

$95 or 1% of salary for 2014

?

$325 or 2% of salary for 2015

$695 or 2.5% of salary for 2016

(1/2 these amounts for each household person under age 18)

Under our current plan year, Lexington R-V School District pays a minimum of $6,540 toward the cost of the medical plan coverage for each covered staff member.
