The BEHAVIOR AGREEMENT and MEDICAL AUTHORIZATION FORM are to be completed and returned to Mrs. Gray BY APRIL 12.. Much advanced planning has been done to assure the most benefit to the students. Every day will be busy and the curfew will be strictly observed. All rules and regulations will be enforced. Students who do not act in accordance with the regulations as stated for the trip will be subject to disciplinary action on the school level. Necessary decisions in this regard will be made by the organization director and the school administration. We hope to have a good time, a beneficial educational experience, perform at a superior level and conduct ourselves in a manner, which will reflect favorably on our homes, school, and community.

LUGGAGE

Each girl will be limited to the following luggage, (1) carry on, (1) other checked large bag or suitcase, (1) Personal item. NO EXCEPTIONS!! Suitcase should be a reasonable size (25” X 18” maximum and no more than 50 lbs) Do not over pack, leave room for souvenirs and if you can get it all in a carry on you will not need to check a bag.

If you check luggage: There will be a charge of $50.00 round-trip for your 1st piece of checked luggage, and $70.00 round-trip for the 2nd. Fees will be collected at the Ticket Counter. Luggage must not exceed 50lbs each or overweight fees will be collected on site ($100.00 one way). The airline does not accept cash, personal or school checks. CREDIT CARDS ONLY! YOU MUST MAKE SURE YOU HAVE A CREDIT CARD OF VISA GIFT CARD FOR $50 TO PAY FOR YOUR LUGGAGE. LUGGAGE HAS NOT BEEN PRE-PAID.

• Do not lock your baggage (unless using TSA approved locks) due to Transportation Security Administration screening of every checked bag.

• Place your name and address on the inside as well as the outside of your baggage

• Carry valuable items such as electronic equipment, cameras, film, cash and jewelry with you on board the aircraft

• Carry necessary items such as medication, prescriptions and keys with you on board the aircraft

• Claim your baggage immediately upon arrival

ID FOR CHECK-IN

Passengers will bring luggage and their photo ID to the ticket counter to check-in. Due to increased security at the airports, each passenger MUST show a GOVERNMENT ISSUED photo ID (i.e. passport, driver’s license or school ID) upon check in.

CLOTHING

See required dress attire on itinerary and pack list.

MONEY

All hotel accommodations, transportation, sightseeing, and tours have been included in the price of the trip. Money will be needed for a few meals, individual expenses, and/or souvenirs. **Limited food service is available on the plane by purchase only with credit card. NO CASH WILL BE ACCEPTED FOR PAYMENT. PLAN ON EATING AT THE AIRPORT ON FRIDAY NIGHT AND TUESDAY NIGHT (THESE ARE BOTH TRAVEL DAYS). You must purchase your food in the airport or bring it from home. Do not bring drinks through security, as they will be confiscated. You will need money for the following things while in New York:

Friday: $50 on VISA GIFT CARD or CREDIT CARD for Baggage, Dinner money at Airport before flight ($10)

Saturday: snack money & money for shopping/souvenirs throughout the day

Sunday: snack money, lunch money ($10), money for shopping/souvenirs throughout the day

Monday: snack money & money for shopping/souvenirs throughout the day

Tuesday: snack money, lunch money ($10), money for last minute shopping & dinner at Airport before flight ($10)

It is recommended that you bring traveler’s checks, a debit card, and small amount of money. DO NOT bring a credit card with some one else’s name. You must have ID in order to cash traveler’s checks.

BUDDY SYSTEM

Do not go anywhere alone, you must go everywhere in groups of THREE ALWAYS!!! NO EXCEPTIONS! New York is a long way from home. If you have a question about leaving the group, see the chaperone in charge of your group. We will not have time to look for any lost sheep. This should even apply to going down to the lobby. Do not cause the entire group an inconvenience by taking this lightly.

CHAPERONES

Adults will be taking the trip as official helpers and chaperones. Their rooms are spaced among the rooms of the Troy-Anns/Cheerleaders. The chaperones have final say in on spot decisions about where your group will go during “free time”. Mrs. Ewart and Mrs. Gray will do room checks/lights out

ROOMS

You may not make room changes. Do not leave any valuables in your room. The hotel will not be responsible for lost or stolen items. Leave them at home. You do not need any expensive jewelry - - you have no place to wear it. One girl will be assigned as room captain for each room. She will be responsible for the following: seeing to it that no one leaves the room after bed check, keeping up with the room key, keeping the room neat and orderly, and reporting any room damages, having meetings with Mrs. Gray and Mrs. Ewart if necessary, voting for her room on decisions if necessary.

HOTEL CHECK IN

Upon arrival at the hotel, everyone waits in the bus or lobby (which ever Mrs. Gray chooses) for room keys and instructions. Room captain will pass out keys to her room. Students should take their key, pick up their luggage and go directly to their rooms as their name is called out. Should anyone need an extra key, it may be obtained form the front desk but should be taken right back for the next time you might be locked out. Should you find upon checking into your room a missing bed, soap, towels, etc., please let your chaperone know at room inspection. To avoid disturbing other guests, students are to be quiet when going from one room to another and when entering or leaving the hotel. The Troy-Anns are responsible for any damages done to the hotel by our group. Please remember that this is a hotel, so please wear a robe or be fully dressed when going into the hallway.

TOURING/SHOPPING GROUPS

You will be assigned touring/shopping groups. Changes may not be made to these groups unless permission from Mrs. Gray or Mrs. Ewart has been obtained. You will exchange numbers with your chaperone. Chaperones must know where you are at all times. You must also stay in your touring groups at all times. No Exceptions!!!

TELEPHONES

If you absolutely have to make a phone call, it must be made from the lobby or your own personal cell phone. No calls - - even collect - - are to be made from your room, since a fee is attached to each call. We don’t want any check out bills. You may make a call from the lobby or your cell phone and give your parents your phone number so that they may call you. There are no exceptions to this rule.

VALUABLES

Never leave any cash or other valuables in your room. Please leave all valuables at home. Neither the hotel nor the travel agency will be responsible for cash or valuables left in the room.

WAKE-UP CALLS

You may make arrangements for a wake-up call. Your chaperones will give you a wake-up call if necessary. Your chaperone will come by to make sure you do not oversleep. Room Captains should also be responsible for making sure no one over sleeps.

ELEVATORS

Please try and minimize use of the elevators and only push one floor -- yours. Many times all floors will be pushed and this will make your trip and that of others take a lot longer than necessary, plus it will tie up the elevator.

SWIMMING

There is no lifeguard on duty, swim at your own risk. Make sure that the chaperones know where you are going before you go to the pool area. Observe the rules posted by the management. Exclude all horseplay, running or any activity that might injure or embarrass someone. DO NOT GO SWIMMING BY YOURSELF OR AFTER DESIGNATED HOURS.

LIGHTS OUT

After bed check, lights are to be out! No exceptions!! You will be given ample time to prepare for bed. Keep in mind that the hotel is a public area and house robes are to be used.

DISCIPLINE

Any discipline problems will be handled on the school level. Any girl not following the rules established will be returned home immediately at the parent's expense.

ALCOHOLIC BEVERAGES – SMOKING – DRUGS

Don’t even think about it! These are forbidden at all times. Consumption and/or possession of, in rooms, on the person, or at any time will be immediate grounds for cancellation of the trip for that individual. Anyone breaking this rule will be subject to disciplinary action on the school level and will be returned home at the parent's expense. Please remember you are representing the Trinity High School Troy-Anns and, more importantly, yourself. Think before you act! You are a visitor in New York and many people will be watching you. We are depending on you to conduct yourself in a manner that will continue to make everyone proud of you. Keep all comments and remarks to yourself to avoid any problems.

MEDICATION

Please fill out the special instructions section on the MEDICAL AUTHORIZATION FORM if you must take any kind of medication - whether self-administered or by our school nurse.

VIDEOS

Only rated G, PG, and PG-13 videos will be allowed on the charter buses. If you have a video you would like to bring, please have it approved by Mrs. Gray prior to the trips.

BE FLEXIBLE And go with the flow!

Many of hours have gone into the planning of this trip, but it is impossible to predict late planes or similar inconveniences. It is not unusual for there to be a need for a change in the schedule after we arrive and become aware of information that perhaps was not available before. Don’t be quick to criticize. Relax and enjoy the trip – a complainer ruins the trip atmosphere for everyone. Give your chaperone the respect they deserve and thank them for volunteering their time.

GET SOME SLEEP

Everyone loves to stay up late. However, on a trip of this length, it is really very important for you to get adequate rest. This should be a learning, fun, and rewarding trip. Get some sleep so that we can enjoy each other, enjoy the trip, stay well and perform to the best of your ability.

AUTHORITY OF ADULTS

Mrs. Gray, Mrs. Ewart, chaperones, employees of the hotel, airline, restaurants, museums, bus line and any place that we go sightseeing, are the people in charge. If they ask or tell you to do or not do something immediately give them your enthusiastic cooperation and attention. No comments are necessary or appropriate. It is their duty to provide supervision. This would include corrections of inappropriate behavior. Be cooperative and remember – without their service there would be no trip.

GENERAL TRIP RULES AND REGULATIONS

1. Be certain you understand the itinerary and follow it at all times.

2. Stay with the group whenever we leave the hotel or buses. Departure from the group must be cleared with the chaperones.

3. Be considerate of the chaperones. They are on the trip to help as well as support your accomplishments. Remember "thank you" and “yes ma'am” are expected – please be considerate, even if it is your own mother.

4. Do not make random acquaintances. No exceptions. Do not tell anyone what hotel we are staying at or invite anyone over.

5. Curfew times are clearly noted. Do not leave your room for any reason after this time. You will be able to phone your chaperone if an emergency arises.

6. Whenever we board the buses, find your seat immediately. Each time we load we must take roll -- so the quicker you get on and get quiet we will be on our way.

7. Smoking will not be allowed: students smoking at will be subject to disciplinary action, such as being sent home.

8. Do not leave any money or valuables in your room at any time.

9. Drinking or possession of alcoholic beverage or drugs will constitute a most severe breach of discipline. Any student allowing herself to become involved will be sent home immediately at the parent's expense. No foolishness will be tolerated at this point. Remember this is a school-sponsored activity (and you must abide by the Student handbook and Troy-Ann or Cheerleader Constitution).

10. Act like a lady at all times.

11. No males are allowed in the rooms at any time. No females are allowed in the rooms of males.

12. If you are changing chaperone groups BOTH chaperones must approve and make Mrs. Gray or Mrs. Ewart aware.

Many people have shown their pride and confidence in you by either giving you financial assistance or contributing to your sales and/or personal effort to make this trip possible. Let us conduct ourselves in a manner, which indicates we are deserving of this confidence. It takes the best in each of us to bring out the best in all of us!

How Much Can I Carry On?

• You can bring one small carry-on bag plus one personal item per passenger as long as the carry-on bag fits comfortably in the sizer without being forced and does not exceed overall dimensions of 45 inches (length + width + height).

• The maximum dimensions cannot exceed any of the following measurements: 22" long x 14" wide x 9" tall or 115cm (56 x 36 x 23 cm). All carry-on items should be stowed in an overhead bin.

• Personal item - includes: purse, briefcase, laptop bag OR a similar item such as a tote that does not exceed 36 inches( length+width+height) and must fit under the seat in front of you.

• Additional items that will be allowed above and beyond the restrictions listed above include:

o Outerwear such as coats/wraps/hats

o Book or newspaper

o Small bag of food to eat on the flight - see guidelines for liquids

o Pillow or blanket

For Security Screening:

• Place liquids in individual containers with a maximum capacity of 3.4 oz./100 ml each.

• All liquids, gels and aerosols in containers larger than 3.4 ounces must be packed in your checked bag.

• Place all liquid containers in a separate clear plastic, zip-top bag no larger than 8in x 8in (20cm x 20cm) or is quart size.

• Items must fit in the bag comfortably so that it closes completely and must be submitted for inspection separately from other carry-on baggage.

• Remove the plastic bag from the carry-on bag at the security checkpoint for separate x-ray screening.

• Exemptions to the liquids rule apply for liquid medicine or baby milk/food. View TSA guidelines for traveling with formula, breast milk or juice.

• Customers carrying liquids in carry-on bags may be subject to secondary searches before boarding.

I, ________________________, understand that from the time the Troy-Anns leave for their trip to New York City, New York on April 22nd, 2016 and return on April 26th, 2016, that Mrs. Gray is in charge. Her responsibilities consist of all decision making, dealing with disciplinary problems, and dealing with any further occurrences that may need her attention and authority. It is my duty to cooperate with them to make it a safe, happy, and rewarding trip. I understand the Assistant Principal Becky Ewart will aid Mrs. Gray in disciplinary action in compliance with the Trinity Student Code of Conduct and Troy-Ann Constitutions.

I realize that I must:

1. Obey all school rules as they appear in the student handbook.

2. Obey all Troy-Anns rules as they appear in the Code of Conduct and infraction system.

3. Remain in groups of 3 at all times. Be on time for check-ins with chaperones, and stay in assigned groups.

I must never go anywhere alone!!

4. Conduct myself in a lady-like manner at all times.

5. Respect hotel property and keep our buses clean.

6. Respect authorities in charge.

I also realize that:

A team meeting will be held periodically to set curfew and give information about the day's events. You are required to attend those meetings. Troy-Anns are expected to be in their rooms at curfew. Tape will be placed on your door at curfew. If you leave your room past curfew there will be no way to replace the tape.

I realize that my behavior on this trip is a direct reflection on my parents, the community, the Troy-Anns, Trinity High School and myself. I also realize that my behavior will have a direct effect on future trips at Trinity High School.

I realize that if I disobey any of the rules and regulations set forth for this trip that my parents will be asked to come get me at their own expense, or I will be sent home immediately at my parent's expense. In this event I also realize that I will lose my membership as a Troy-Ann and the future opportunity to be a Troy-Ann. Further discipline will be administered by school officials.

My child MAY/ MAY NOT swim in the hotel pool. (circle one) There is no lifeguard on duty. Swim at your own risk.
