Q.
We are currently coding Steal Syndrome as follows: Steal syndrome is a well-described complication of arteriovenous fistulas (AVF) used for haemodialysis access. Steal Syndrome NEC = I77.8 Subclavian Steal Syndrome = G45.8 (See Index) If it is postprocedural (i.e. due to the AVF) = I97.8 & I77.8 Can you please clarify if we are coding this syndrome correctly by assigning the above codes?

Clinical input was sought. Steal syndrome in general refers to vascular insufficiency. In the context of renal dialysis patients, Steal Syndrome is vascular insufficiency (both arterial and venous) that has occurred as a complication of arteriovenous fistula (AVF). It is not a complication of the AVF itself, rather the complication will only occur in the presence of an AVF. Steal syndrome due to AVF should be coded via the following pathway: Complications -arteriovenous fistula or shunt, surgically created --specified NEC T82.8 To add further specificity we look up arterial and venous insufficiency and obtain the following codes: Insufficiency - arterial -- peripheral I73.9 = I73.9 Peripheral vascular disease, unspecified Insufficiency - venous I87.2 = I87.2 Venous insufficiency (chronic)(peripheral)

I73.9 code description does not provide any further specificity to T82.8 Other specified complications of cardiac and vascular prosthetic devices, implants and grafts and is therefore not added.
