Position Description

As a direct support professional, your primary responsibility is to provide a continuum of quality, compassionate care while meeting the physical, social, and independent needs of those we serve. On a daily basis, your job will be to perform direct client care and other duties that are assigned. This assures that all proper safety measures and body mechanics are followed in delivery of all cares. Staff must document and report all cares given and relevant observations to lead staff or management members.

Job Responsibilities

Assist physically and cognitively disabled clients with daily tasks and activities

Provide personal care to all day center clients o maintain memory o hygiene o toileting o dressing o hair care o oral hygiene o skin care o medication

Provide nutritional meals in accordance to the daily meal calendar Assist with light housekeeping

o laundry o making beds o keeping client activity area neat and clean Assist client with walking and exercising Provide outing transportation and supervision Provide emotional and psychological support to clients through the use of o companionship o conversation Provide clear and concise documentation of progress notes, client charting, and attendance When community outings are scheduled staff will be responsible for attending and held responsible for acting in a professional manner while interacting with clients.

Keep consistent communication with the Director/Owner of the Coulee Region Adult Day Center and report any concerns or ideas to management immediately

Other duties may be assigned as necessary.

Physical: Must be able to operate all equipment (ex. Wheelchairs, lifts, buses etc.) Must be able to exert up to 50lbs Must be able to twist, bend, push, and pull objects Must be able to stand, walk, and reach above your head
