← After 3 Years, physicals are only one time per year.

Scheduling Policies

➢ No walk-in physicals

➢ Routine Well Physicals for children older than 6 months should be made 2 weeks in advance.

➢ No 15 minute grace period anymore.

Cancellations

➢ If you do not call to cancel within 24 hours of your appointment time, you will be charged $25.00

➢ If you miss your appointment more than 3 times, a warning letter will be sent to your home.

Relatives/Friends

➢ Parent(s)/Guardian must bring child. Please plan ahead.

➢ Children brought by friends or relatives must have the guardian’s written consent before we see them.

(Persons Up to 200 lbs*) $90

* For persons >200 lbs, please ask for pricing.

Travel vaccines and ear piercing must be paid with cash or

Credit card (No Discover).

Co-Payments & Balances

➢ Co-Payments and balances are due before you go into the exam room.

➢ Co-payments maybe applied if Well child visit if combine with a non-routine visit.

➢ Co-pay $10 Late Fee if not paid on visit day
