6. BRIEF RESUME OF THE INTENDED WORK: 6.1 NEED FOR THE STUDY :

Shodhana therapy is indicated in santarpanajanya vyadhis. Acc. to Acharya Sushruta Virechana corrects agni and causes stability of dhatus 1a . Vasti is considered as the best remedy for morbid vata. By using various combinations of ingredients, it is considered good even in other doshic involvement and beneficial in diseases of all the three paths where most of them are due to involvement of vata, thus said to be ardha chikitsa.2a

Due to advanced technology and busy schedule , the lifestyle of an individual has become sedentary along with lack of exercise and there is increased popularity of fast foods leading to impairment of metabolism in an individual making him prone to series of disorders called as lifestyle disorders.

Hyperlipidaemia is one such disorder where there is abnormally elevated levels of lipids in the blood .It is identified as potential risk factor for development of coronary heart disease. In India persons suffering from CHD have increased in last 20 years. In South.India CHD incidence is 7.4% in rural areas and 13.9% in urban areas .According to WHO raised serum cholesterol levels is one of the top ten causes of death throughout the world.

Several drugs are available for the management of Hyperlipidaemia where most of them are potentially toxic, costly and are contraindicated in hepatic or renal impairement, gall bladder disease and pregnancy. Ayurveda can intervene by modifying the risk factors aiming at the prevention.

There is no direct reference available in our classics correlating hyperlipidaemia. It can be included under santarpanajanya vyadhi as medo roga. It is a condition caused by derangement of agni, leads to amarasa, there is medodhatvagnimandya leading to improper formation of medo dhatu in excess and if not arrested further results in sthoulya.

Bhadradi asthapana basti by virtue of its lekhana dravyas reduces medas and pacify the vata by affecting its seat of pakvashaya. Virechana with ashwakanchuki rasa with its ruksha teeksna properties does srotoshodhana. So here an attempt has been done to evaluate the role of virechana and basti karma to compare the effect of both in medo roga.

6.2 REVIEW OF LITERATURE:

• Acharya charaka has mentioned use of Virechana as one of the treatment procedures in santarpana janya vikaras.

• While explaining chikitsa for medoroga, Vangasena includes lekhana basti as prime treatment for medoroga.

• Bhadradi asthapana basti has been specifically indicated in medo roga according to acharya Sushruta.1b

• No separate disease mentioned in brihatrayees in the name of medoroga, but atisthoulya has been explained which can be considered under medo roga.2c

• Features of medo roga has been clearly mentioned by acharya Madhava under separate chapter of medo roga including its nidana, samprapti and lakshanas.

• References of medo roga is also available in Sharangadhara samhita7, Bhavaprakasha8 ,and Yoga ratnakara9.

Modern review:

Hyperlipidaemia or hyperlipoproteinemia is the condition of abnormally elevated levels of any or all lipids and/or lipoproteins in the blood . it is the most common form of dyslipidaemia.10

It may be basically classified as primary (familial) which is basically due to genetic predisposition and secondary (acquired ) when resulting from other underlying disorder that leads to alterations in lipid and lipoprotein metabolism.

Treatment includes Non pharmacological :exercises and reduced intake of saturated fats.

Pharmacological: HMGCoA reductase inhibitors(statins)

Cholesterol absorption inhibitors(ezotimibe)

Fibrates11

6.3 OBJECTIVES OF THE STUDY:

1. To evaluate the efficacy of Virechana karma in medo roga.

2. To evaluate the efficacy of Bhadradi asthapana basti in medo roga.

3. To compare the efficacy of Virechana and Bhadradi asthapana basti in medo roga.

7. METHODOLOGY:

7.1 Source of Data:

For the present study patients are randomly selected from the OPD and IPD of Shri Jayachamrajendra Institute of Indian Medicine Hospital, Bengaluru-09

7.2 Method of collection of data :

30 patients who fulfill the inclusive criteria will be selected randomly irrespective of sex, religion, occupation and economic status.

A) Diagnostic criteria:

Diagnosis will be based on

• Clinical features of medo roga like ashaktah sarva karmasu, kshudra shwasa, swedadhikya, utsaha hani, anga gaurava as mentioned in classics.

• Serum lipid profile higher than normal.

B) Inclusion criteria :

• Aged between 18-60 yrs.

• Having clinical features of medoroga

• Serum lipid levels more than normal ranging from : S.cholesterol(201mg/dl and above)

S.Triglycerides (161mg/dl and above)

Serum LDL(131mg/dl and above )

Serum VLDL(41mg/dl and above)

• Fit for virechana and basti karma

C) Exclusion criteria:

• Associated with any systemic diseases and cardiovascular diseases.

• Associated with any rectal pathology like haemarrhoids, fissure etc.

• Associated with endocrinal pathology.

• Not fit for virechana and basti karma .

D) Plan for the treatment:

A minimum of 30 patients randomly divided into 2 groups A & B with 15 patients in each group

STUDY DESIGN – A prospective open uncontrolled randomized comparitive clinical study

GROUP ‘A’ = VIRECHANA KARMA IN 15 PTS.

Duration of treatment in group A is approximately 12 days. May vary slightly according to agni and koshta of patients.

GROUP ‘B’ = BASTI KARMA IN 15 PTS

Poorva karma : Deepana pachana with trikatu churna (1-2gms bd) for 3 days . If ama pachana not achieved then continued until niramavastha. Before administration of basti, sthanika abyanga and sweda is given to the patient.
