Affidavit for Lost/Surrendered Driver's License, Beginner's Permit, or Identification Card

In accordance with SC Code of Law 56-1-20, an individual is not permitted to have more than one credential at any time. A South Carolina resident is only entitled to have one state issued credential. All prior issued credentials are invalid. For example, you cannot hold a driver license or beginner permit along with an identification card.

Section A: This section is to be used when your driving privileges are not suspended or you are requesting an identification card.

For Example, you are requesting: (1) a driver's license or beginner's permit and your driving privileges are not suspended; or (2) an identification card and either: a) your driver's license or beginner's permit is suspended, or b) you do not have a driver's license or beginner's permit.

I am requesting replacement for a lost, stolen, or misplaced driver's license, beginner's permit, or identification card. I certify, under penalty of perjury, that:

1. I have not surrendered my driver's license or beginner's permit in court following a conviction in court and that I have not surrendered my driver's license or beginner's permit to a law enforcement officer because of an implied consent suspension.

2. I have attempted to locate the identification card(s), driver's license or beginner's permit that was previously issued to me and do not have any knowledge of where it may be located. If any previously issued driver's license, beginner's permit, or identification card(s) is found, I will forward it to the Department of Motor Vehicles immediately.

3. The above information is true and correct. I understand that if this information is found to be fraudulent, I will be prosecuted and the driver's license, beginner's permit, or identification card(s) issued will be cancelled.
