Quick Start Instructions
If the notes do not appear to open correctly, make sure that you have the latest version of Microsoft Word. If you do not have Microsoft Word, you can get a 60-day trial version of by going here.
If you do not want to download Microsoft Word, you can use OpenOffice Writer, a free word processor. To get the OpenOffice suite, go here.
Another option is to use Google's word processor. Please make sure conversion is turned on in order to edit the notes.
Please note: our notes look the best in Microsoft Word and may have errors (such as additional spacing) in the other programs. We are working to improve this.
Once you've opened the note, customize it to your needs. Some notes are more customizable than others. Some require filling in info by hand. Others can be fully edited on the computer.
What You Must Do Before Printing Our Notes
Before printing our notes, be sure that you are using a very high quality printer. We recommend using an inkjet or, better yet, a color laser printer. A color laser printer is available at Kinkos or your neighborhood print shop. Simply go to the print shop and open your email and print the note there.
Its also critical that you print the notes using the highest level of print quality. This can be adjusted by clicking File > Print > Properties > Print Quality ? High. If you do not see these options, just be sure to set the printer on the highest DPI possible.

We do not want to rip anyone off. However, we ask that you respect our product and respect us. We do not wish to freely give out refunds to anyone who just wants to look at our product. Our notes have taken a great deal of research and work.
Also, due to our notes being a digital product, it is very hard for us to know if you've simply downloaded our product, put it to use... and now want your money back.
By default, we trust you and that you will not let us down. But because of this, we have very strict rules on our refunds.
To receive a refund, please DO NOT contact your Bank. This is a massive hassle and will take a great deal of time to receive your refund. It will also hurt our business and cost us money. It will result in you being banned from our site permanently.
Instead, simply email us at help@ and discuss the matter with us.
With your refund request, we ask that you please provide a THOROUGH explanation of why our notes did not work out for you.
We will not refund you without a CLEAR and DETAILED explanation.
Our notes are indeed modeled after real notes. To say that they do not look real is NOT a valid excuse. 99% of doctor's excuses are NOT complicated in their appearance. Most people looking for our notes are simply looking for the layout, and we've provided this to you.
If you have doubts, simply look at the note called the "Comparative Note" . This is a scan of a note that we received from a customer. If you scroll down, you'll see a note we made to mimic it. They look very similar. You can use the 2nd note for your own use.
We're sorry that we have to be very strict in our refund policy. This is a result of many people abusing our system.

"What Note Do I Use For ____________?"
Many customers email us and ask which note to use. We really cannot respond to these types of questions --- as it takes time and we really cannot advise you on which notes to use.
The rule is this: every clinic (in the USA) has a unique note. Every clinics notes look different. That is why its usually pretty safe to use whichever note you feel most comfortable with. Please feel free to customize the note and tailor it to fit your needs. You can cut and paste from all of the notes to make your own custom note. There is no risk in doing this as there is no standard 'doctors note'.
However, our package includes two types of notes: General notes for general hospital / doctor visits, and notes from specialty doctor visits.
