Notes:

II. Only authorized purchasers included in the State of Washington Purchasing Cooperative (WSPC) listings published and updated periodically by OSP may purchase from this contract. It is the contractor’s responsibility to verify membership of these organizations prior to processing orders received under this contract. A list of Washington members is available on the Internet , contractors shall not process state contract orders from unauthorized users.

III. Contract Terms: This Document includes by reference all terms and conditions published in the original IFB, including Standard Terms and Conditions, and Definitions, included in the Competitive Procurement Standards published by OSP (as Amended).

Special Conditions:

1. 07/15/05: please reference the attached for revised pricing effective July 22, 2005. Changes in pricing, stock numbers, etc have been bolded.

2. 05/20/05: Fisher Scientific will be submitting a revised pricing schedule for review/approval within thirty days which will impact the decision to extend this contract for the duration of the proposed two year extension.

3. 01/03/05: please reference the attached price list for new stock numbers, and recommended shelter configurations by the COT group. Also, please note:

a) revised manifold stock numbers (due to specification enhancements) and pricing.

b) the copper hand sprayer has been discontinued (replaced with the brass hand sprayer)

c) the small shelter added in June, 2004 has been temporarily removed for the contract

d) new item #19 added (backflow check valve) in response to user request

4. 06/23/03: add additional item per customer request (smaller decon shelter). Reference item 18,

Price Sheets. Also, correct Fisher Scientific’s fax number as indicated.

5. 05/23/03: This is a new contract developed at the request of the Military Department and will be utilized to implement the federally funded “Homeland Security” project within the State of Washington.

6. Training requirements: supplier is required, to the extent possible and based upon mutual agreement with the purchaser(s), to provide training on how to utilize products specified herein. This can be accomplished by a method as simple as a video or well detailed instruction manual, or if deemed necessary, an on-site, hands on training of agency personnel. Training shall be provided at no additional cost.

Supplier has noted the following in regards to training: “Fisher/TVI will support hands on training for the first responders of your trainers if you prefer with a five day consecutive day blitz. The state would need to coordinate the locations and consolidate the students to five locations (one per day). Additional training classes or an on-line training course are available for purchase. Please note that TVI provides initial product fielding and familiarization training in conjunction with full-system purchases. The training, typically provided in conjunction with delivery and fielding of the purchased equipment and components, but is subject to alternative scheduling, if more convenient to the client (note: alternative scheduling of training may be subject to transportation and billeting costs). TVI’s inclusive training module is a one-day training program (not to exceed ten hours) entitled “Introduction to the TVI Decontamination System”. Training is based on the TVI Operator Manual and features the following functional areas:

System and component description and overview

• Site/location and preparation

• Set-up/installation

• Pre-operation checks and services

• Operations

• Post-operation, teardown and stowage

7. Emergency Purchases: supplier agrees, to the extent possible, to honor requests for “emergency purchases” Any additional costs incurred for special delivery will be borne by the purchaser. In the event supplier is unable to meet such requests, purchaser(s) will be authorized to purchase their immediate requirements from non-contract suppliers.

SPECIFICATIONS – DECONTAMINATION SHELTERS

Shelter features:

• Articulating structural members

• Interior shower basin

• Structural members of 7000 series modified structured aircraft aluminum on an articulating lateral quad frame structure, constant tension systems are not acceptable

• Capable of accommodating side entrances

• Capable of maintaining structure under still conditions with 30% of frame members missing or broken

• Integral plumbing with spray nozzles designed to give .6 gpm cone spray

• Integral white roof to allow for the passage of natural light and to enable exterior flood lighting at night

• Equipped with a thermal screen to reduce internal temperature build up

• Integrated floor and anchoring system must be able to be picked up and repositioned by four people while deployed and assembled

• Clear internal span without internal supports

• Exoskeleton to reduce the likelihood of internal obstruction and simplify clean up

• Exterior canopy to be Aeron 27, vinyl coated nylon, fire retardant (cal 83), UV stabilized and chemical resistant

• Proven experience of wind loading in excess of 50 mph

• Completely decontaminatable in a hypochlorite solution

• Severe weather straps on each of the external lateral nodes

• Hydrostatic Resistance: (CCC-T-191 method 5512) the high range hydrostatic pressure resistance is not less than 25 lbs. Per square inch

• Oil Resistance: shows no leakage when lubricating oil conforming to MIL-L-6082 is held in a pocket of the coated cloth at a depth of 1/2 inch for one (1) hour

• Hydrocarbon Resistance: does not crack when creased after a five minute immersion in a hydrocarbon fluid conforming to Type II of Mil-H-3136

• Mildew Resistance: (CCC-T-191 method 5762) has good resistance to mildew and does not appreciably stiffen nor have more than a 10% loss in breaking strength after the fourteen day mildew resistance test

• Flame Resistance: (CCC-T-191 method 5902) shows an average time of flame of not more than 12 seconds and average length of char of eight inches maximum when tested in accordance with the vertical flame resistance of cloth test (Far 25.853 Para. B)

Canopy UV stabilized, fire retardant and chemical resistant. Shelter constructed of non-corrosive material and includes transport bag, ground cloth, utility poles, repair and anchor kits. 10.5' wide x 19.5' long, 215 pounds, 45 inch stored cube size.

Lights: shall be capable of giving 3150 lumens each, have a 120V rating with Nema 15-5 plug, with the following features:

• fixture lengths to be 32.5" with a PL 40/41 bulb type with a low temperature start capability of 32 degrees fahrenheit

Non-ambulatory litter conveyor and transfer board system: shall be collapsible table conveyor system for easy movement of non-ambulatory patients. Rapidly deployable, lightweight and rugged. Conveyor system shall provide the following features:
