Haematology History

Recent infection: HUS, ITP

Sx of anaemia: SOBOE, fatigue

Jaundice

Sx of CT disease: joint pain / swelling = sickle cell

Leg ulcers = sickle cell, spherocytosis

Abdo / back pain = sickle cell, paroxysmal nocturnal haemoglobinuria

= gallstones / cholecystitis in haemolytic anaemia

Neuro: spinal cord lesions = spherocytosis, haemolytic anaemia, lymphoma

CVA = sickle cell

Fluctuating = TTP

DH: drugs cause haemolytic anaemia

Anticoagulation

Thrombotic prophylaxis

PMH: operations (eg. mechanical heart valve)

PMH of haemolytic episodes – sickle cell, spherocytosis

Thrombotic tendancy

Unexplained miscarriages = antiphospholipid syndrome

MI = factor V Leiden

FH: thrombotic tendancy

SH: ethnic background

RF for thrombosis (eg. smoking, OCP, pregnancy, malignancy, recent surgery,

immobility)

Haematology Examination

Skin: Bruises (ecchymoses, petechiae) = thrombocytopenia, scurvy

Pigmentation = lymphoma, Fe overload in thalassaemia

Rashes = lymphoma

Ulceration = neutropenia

Cyanosis, plethora = polycythaemia

Jaundice = haemolysis

Scratch marks = myeloproliferative, lymphoma

Palpable purpura = vasculitis, dysglobinaemia, bacteraemia

Hands: koilonychia = Fe def anaemia

Pale palmar creases = anaemia, Hb ................
