Now that you have determined if the applicant meets the program and project requirements (income, age, and/or disability) for the community, you will complete the verification process.

Verification of Disability (Form C-19)- You must tell the applicant that HUD permits owners to verify that they have a disability only if 1, their eligibility for admission is dependent on the applicant being a person with a disability; or 2, applicant claims eligibility for deductions that are given to a person with a disability.

The definitions of disability vary depending on the project applicant is applying for or living in. The owner determines the definition(s) to use by consulting with HUD Handbook 4350.3 REV-1. The third party from whom this verification is being requested has knowledge of whether your disability meets the applicable definition(s) of disability (or person with a disability). An owner may request from a third party only the minimum information necessary to determine whether you meet the applicable definition of disability (or person with a disability). Any other request for information about you is not relevant and may not be asked (e.g., diagnosis, treatment plan).

"NOTE: This information may have to be conveyed in languages other than English for LEP persons in accordance with HUD guidance."

Included in the forms section of the manual are two sample formats from HUD. Please note that if Item 4, on either sample format, is checked “YES”, the applicant/tenant does not meet HUD’s definition of disability.

Statement of No Income (Form O-02) - The amount of rental assistance paid on behalf of the household is calculated using their annual income, less allowable deductions. Therefore, it is critical that you obtain and adequately verify annual income and benefit information in making rental housing subsidy determinations.

Through the certification and recertification process, O/As are to make sure that all residents receiving assistance, pay rents which is equal with their ability to pay. During this process, if certification and verification indicates that there is no income or very sporadic income, then it is this company’s policy to ensure that residents pay the correct rent and receive the correct HUD assistance.

This process requires that the resident is required to supply the following information every ninety (90) days so that we can ensure that no income is overlooked. All responses are subject to verification. Incomplete forms will not be processed which means that assistance and/or tenancy may be denied or terminated as appropriate.

Once a family/household has been determined to be at zero income, their income will be reverified once a quarter (every three months). Make a note or reminder indicating that the household is reporting zero income and that follow up needs to be completed.

If, after three months the household is still at zero income, follow up is initiated. During this quarterly follow-up, the resident must complete the following documents which are subject to verification:

• Zero Income Questionnaire (Form O-01);

• Statement of No Income Certification (Form O-02);

• Unemployed Affidavit (Form O-3)

• Recurring Gifts/Financial Assistance Certification (Form O-04)

• 90 Day Tracking of No Income (Form O-05)

• Any, additional certifications or verifications required

Verification of Medical Expenses-(Form C-10){legal size paper} Make sure you have only 12 months of prescription data, make sure you are only counting medical expenses from the previous 12 months or the up coming 12 months, If a doctor or pharmacy predicts the up coming 12 months USE THESE FIGURES instead of the prior years. If a pharmacy does not supply you with a full 12 months of expense you must annualize to get a full 12 months expected costs. You only need to verify medical expenses that the applicant or resident pays for out of pocket.

Verification of Employment- (Form C-11){legal size paper} If applicant has a job, then we need to know how many hours Applicant will AVERAGE per month during the coming year and the hourly rate the applicant is paid. Remember there are 52 weeks in a year if you get 4 weeks of income verification you can not use that for one month of income.

Verification of Social Security- (Form C-12){legal size paper} If SS is 620.90 but gross pay is 620.00 you still use 620.90. If the SS amount is $620.90 and the gross pay is $542.70 and the Medicare amount is $78.20 than you would still use $620.90 for SS amount but then give medical expense allowance of $78.20 per month in the medical expense section. For new move ins you would obtain verification from Social Security Administration office. For existing tenants you must use the EIV system to obtain verification.

Verification of Pension- (Form C-13){legal size paper} Verify with company that applicant is receiving pension from. Verify the gross amount received before any deductions. This is the amount that would be counted as income. Then verify if the applicant has any medical deductions that are taking out of his pension amount. If yes, than you would count the medical expense as a medical deduction. YOU DO NOT REDUCE THE GROSS PENSION AMOUNT.

Verification of Real Estate- (Form C-14){legal size paper} Either contact the Real Estate Tax Assessors office for value of land and property or have the applicant supply you with the documentation of values. The easiest thing to do is to use the internet, you can obtain most of this information through online tax assessor’s county websites. Also must deduct principal balance of the mortgage and deduct 10% of value to cover cost of selling.

Verification of Assets- (Form C-15){legal size paper} Checking use 6 month average, Savings use current statement. Both account types require verification of interest.

In the event that you are unable to verify third party checking, you must complete the checking account worksheet and attach to the top of the 6 consecutive bank statements. (Form C-16)

All data must be verified in writing by a third party.

In addition to verifying the above verification forms you must verify all other data supplied to you on the application. Such as:

Verify Social Security Number: Obtain copy of Social Security Card or print out from Social Security that contains the Social Security Number

Verify Date of Birth: Obtain a copy of Birth Certificate or print out from Social Security that contains the date of birth. Note, if applicant supplies ID card or Driver’s License please compare the date of birth on this document with the birth certificate/print out from Social Security office. If different, research and determine which one is correct and document the file. If date of birth does match the Social Security Office have resident contact SS office to correct.

Verify if Citizen or Eligible Non Citizen: An applicant for HAP Property must be a United States Citizen or an eligible noncitizen. To determine if applicant is eligible non citizens refer to HUD 4350.3 Chapter 3, Section 12.

HUD Form 92006 Supplement to Application: This form is to be given to each applicant to complete. Applicants cannot be required to complete this form as it is not a requirement for occupancy. If the applicant chooses not to complete this form than they must sign, date and check the box located on the HUD Form 92006 which states the following:

Check this box if you choose not to provide the contact information.

At any time applicant/resident may update, remove, or change the information they provide on this form. And, that they are not required to provide the contact information, but if they choose to do so, they are to include the relevant information. However, the community manager should encourage the applicant/resident to complete this information. The purpose of the contact information is to identify a person or organization that may be able to help in resolving any issues that may arise during the applicant’s tenancy or to assist in providing any special care or services they may require.
