KINDERGARTEN ROUNDUP

Dear Parents:

If your child will be five years old on or before October 1, 2014 he/she is eligible to attend kindergarten. You will be contacted by the Central Registration Office with an appointment date and time for you to register.

Representatives from your school will be available to work with you and your child during Roundup. On the day of registration, please bring the following:

1. Your child

2. Child’s birth certificate

3. Immunization records certified by a doctor or clinic

4. Registration forms completed

5. Dental examination form completed

6. Doctor’s Physical Examination Form completed

7. Health assessment form completed

8. Proof of Residency

During kindergarten registration, proof of residency is required to determine eligibility for admission. Representatives from the Central Registration Office will be present to accept proof of residency.

THE FOLLOWING DOCUMENTS ARE PROOF OF RESIDENCY:

1. Deed or Property Tax Bill or Settlement Agreement or Mortgage (Homeowners)

2. Lease / must be current – (Rental Properties)

3. Notarized letter from landlord if a new lease is not obtainable. (Information provided must include all residents. We will contact/verify with the landlord.)

Also, the Board of Education requires FOUR (4) additional documents showing your name and current Ewing address.

Examples:

• driver’s license

• car registration

• car insurance card

• any utility bills – cable, telephone, electric, (ANY BILLS)

• letter from an employer (showing address)

• letter from bank/bank statement (showing address)

• ANY current business mail showing the parent’s name and current address

It is recommended that each child have a physical examination by a family physician or pediatrician as that doctor has a full medical history and records of your child. A current physical is one that is under 1 year old. If your child is due for a physical (usually around the 5th birthday), you can provide the record to us once it is done (before school starts).

If for any reason this is not possible, please inform your school nurse at Round-up.

The process of Kindergarten Roundup includes:

• Registration/Residency Verification

• Kindergarten Readiness Skills Assessment

• Confirmation of Immunization Requirements/Health Records Screening

Ewing staff members are looking forward to meeting you and your child. All schools will schedule a school orientation. You will be contacted for Kindergarten orientation (a day you and your child visit your school and meet the kindergarten teachers).
