Be aware student pictures may still appear in large group or school pictures and videos with no personal identifier.

MEDIA/DIRECTORY/WEB INFORMATION RELEASE

Please note the Ewing District does not publish photos with residential addresses, e-mail address, phone numbers or locations and times of class trips of its students on the school or district web sites.

Check one choice for Media/Directory/Web Release (See reverse for explanation before choosing and signing):

YES, I/We DO GRANT PERMISSION for the release of media/directory/web information. This includes the permission of photo/images of the student with his or her name without any other personal identifiers to be published in the media (both print and web) and on the school and/or district's public Internet site.

NO, I/We DO NOT GRANT PERMISSION for the release of media/directory/web information. I/We do not grant permission for a photo/image that includes the student to be published in any media outlet (both print and web) or on the school and/or district's public Internet site. We are aware students may still appear in large group pictures with no personal identifier, such as but not limited to concerts, awards/recognition ceremonies, and programs aired on Channel 19.

The notice to parents/guardians will be sent once in elementary school, once in middle school, and once in high school. Parents/Guardians have ten (10) days from receipt of the release to object. If you do not respond within ten (10) days, your child will be granted permission for media/directory/web release. If you wish to change any permission previously given, you may do so at any time by notifying the principal of your child's school in writing.

WEB and ALL MEDIA/DIRECTORY RELEASE INFORMATION

The Family Educational Rights and Privacy Act (FERPA), a Federal law, requires that THE EWING PUBLIC SCHOOLS, with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child's education records. However, THE EWING PUBLIC SCHOOLS may disclose appropriately designated "directory information" without written consent, unless you have advised the District to the contrary in accordance with District procedures. The primary purpose of directory information is to allow THE EWING PUBLIC SCHOOLS to include this type of information from your child's education records in certain school publications. Examples include:

- A playbill, showing your student's role in a drama production - The annual yearbook

- Honor roll or other recognition lists - Graduation programs

- Sports activity sheets such as for wrestling, - Award/Recognition Ceremonies

showing weight and height of team members. - Concerts and other Channel 19 Tapings

Directory information, which is information that is generally not considered harmful or an invasion of privacy if released, can also be disclosed to outside organizations without a parent's prior written consent. Outside organizations include, but are not limited to, companies that manufacture class rings or publish yearbooks or media publications. In addition, two federal laws require local educational agencies (LEAs) receiving assistance under the Elementary and Secondary Education Act of 1965 (ESEA) to provide military recruiters, upon request, with three directory information categories - names, addresses and telephone listings - unless parents have advised The Ewing Public Schools that they do not want their student's information disclosed without their prior written consent.

If you do not want THE EWING PUBLIC SCHOOLS to disclose directory information from your child's education records without your prior written consent, you must notify the District in writing by September 12, 2013. THE EWING PUBLIC SCHOOLS has designated the following information as directory information:

- Student's name - Date and place of birth - Participation in officially recognized activities and sports

- Address - Major field of study - Weight and height of members of athletic teams

- Telephone listing - Dates of attendance - Degrees, honors, and awards received

- Electronic mail address - Grade Level - Most recent educational agency or institution attended

- Photograph

We are sending this parental consent form to both inform you and to request permission for your child's photo/image and personally identifiable information to be published on the district and/or school's web site.

As you are aware, there are potential dangers associated with the posting of personally identifiable information on a web site since global access to the Internet does not allow us to control who may access such information. These dangers have always existed; however, the schools do want to celebrate your child and his/her work. The law requires that we ask for your permission to use information about your child.

Pursuant to law, we will not release any personally identifiable information without prior written consent from you as parent or guardian. Personally identifiable information includes student names, photos or images. Please note the Ewing District does not publish photos with residential addresses, e-mail address, phone numbers or locations and times of class trips of its students on the school or district web sites.

Once you have given signed permission for the above options you may change your mind. As the parent or guardian, wishing to change this agreement, you may do so at any time in writing by sending a letter to the principal of your child's school. The change will take effect upon receipt by the school.

These laws are: Section 9528 of the ESEA (20 U.S.C. 7908), as amended by the No Child Left Behind Act of 2001 (P.L. 107-110), the Education Bill, 10 U.S.C. 503, as amended by Section 544, the National Defense Authorization Act for Fiscal Year 2002 (P.L. 107-107), the legislation that provides funding for the Nations' armed forces.
