- GCS

If your results are abnormal (not 15), you need to relay in report which

component is less than expected. Serial reassessment is not possible without

this piece of information.

Trending is more important than any one reading.

- Rhythm strips and 12 lead EKG’s

If these are obtained in the field, you must add your interpretation to the

Documentation in the patient care run report.

If your 12 lead EKG does not print a 6 second rhythm strip, you must also

attach a rhythm strip with a 12 lead EKG to the patient’s medical record.

This is not new - this has always been a part of documentation!

- Norovirus

Highly contagious viral disease and is found in stool and in vomitus. It is spread via the

fecal/oral route).

Transmitted via contaminated patient, surfaces, food (very common source), or water.

Causes abdominal pain, nausea, vomiting and diarrhea.

Prevention - Contact precautions (use of gloves; gown if necessary). Hand washing must

be done with soap and water, not just alcohol based products. Routine cleaning then

disinfecting must be done with bleach solution and bleach wipes.

- C-Diff

A bacterial infection that can cause diarrhea associated with use of antibiotics. The

bacteria can survive on moist surfaces up to 6 hours. Spores can survive for months or

years on contaminated surfaces. Shed in feces and transmitted via contact with

contaminated hands and surfaces. Causes watery diarrhea, fever, loss of appetite,

nausea, abdominal pain and tenderness.

Prevention - Contact precautions (use of gloves; gown if necessary). Hand washing with

soap and water; not just alcohol based products. Routine cleaning and then disinfecting

must be done with bleach solution or bleach wipes. [pic][pic][pic][pic]
