Any other considerations are secondary to this objective. It is not the policy of Killark to ask employees to remain and attempt to control the situation rather our policy is to immediately evacuate the building.2.0 PROCEDURE: FIREIn the event of a fire, the employee who first notices the fire will immediately notify a supervisor, state his/her name, the nature and location of the emergency. The supervisor will:2.1 Notify the appropriate member of management of the emergency situation. Normally this would be Steve Kerr the Facilities & Safety Manager, Mike Collins the Operations Manager, Warren Jemkins the Business Unit Vice President or if they are unavailable, any other quickly available manager at the facility.2.2 The supervisor will then call 9-911 to report the emergency and location.2.3 During regular business hours, Lori Sharp, HR Manager will call 911 via cell phone to confirm that emergency services are responding to Killark.2.4 Outside of regular business hours, the highest level manager on site will call 911 to confirm that emergency equipment is responding to Killark.3.0 PROCEDURE: BUILDING EVACUATION The entire building is to be evacuated immediately when personnel are instructed to do so via the Audible Siren Alerting System.The following rules are to be strictly adhered to in the event of a building evacuation: 3.1 Supervisors/Managers will make provisions for the safe evacuation of disabled visitors and/or disabled employees.3.2 Close all private offices, hallway and building connecting doors. Leave lights on so people can see to safely exit the building. Open East Lot gates (2).3.3 Proceed to the nearest safe exit. Exit the building in a quiet, orderly fashion.3.4 After exiting the building, all employees will assemble at their predetermined locations in the parking lot to allow verification of a total evacuation. At this time all Supervisors/Managers will account for each of their employees to make sure no one is still in the building.3.5 First aid team members will first report to their supervisors for attendance and then to the Evacuation Coordinator, Safety & Facilities Manager or Upper Management in the vicinity of areas A & B to provide first aid as needed. If possible, First aid team members should exit the building with their first aid kits.3.6Floor Captains will assure areas are totally vacated.3.7 Supervisors/Managers will immediately report their employee status to the Evacuation Coordinator (located at areas A&B), including reporting any missing or injured personnel. In no case should any employee re-enter the building to search for missing personnel. 3.8 Any visitors or contractors in the plant should remain with their host employees and exit the building with that department.3.9 Facility management and maintenance will conduct the following activities provided the emergency doesn’t put their safety at risk:Unlock exterior doors to ease entrance of the fire department personnel. Shut main gas valve in Fire Sprinkler Pump Room.Verify that the sprinkler riser supply valves are open and that the sprinkler system is functioning.Switch off the electrical supply at the main panels after everyone has evacuated the building.3.10 All personnel shall then await future instructions to be given by facility management personnel. Employees must remain away from the building and any responding emergency equipment and personnel. They may not enter or move their automobiles unless instructed to do so.3.11 The Evacuation Coordinator will provide emergency response personnel the following information:Details of any missing personnel and their probable location.Status & location of the emergency.The existence and location of the emergency generator.A diagram of the facility with potentially dangerous areas noted.The location of the MSDS sheets for the facility.3.12 Meeting Places:Upon evacuation from the building, all employees shall stage in Area A (Large Parking Lot across MLK Drive) and Area B (Small Parking Lot across MLK Drive) according to the plan below:Area A: Office / Factory Lighting & Machining / Test Lab employees and VisitorsArea B: Factory Motor Control / West Warehouse employees 4.0 PROCEDURE: ALARM TESTS & DRILLS4.1 The evacuation alarm shall be tested on the first Monday of the month for a minimum of thirty (30) seconds.4.2 An evacuation drill shall be conducted at least once during each calendar year. Snow and Winter StormsBackground: Snow and severe winter weather including ice and sub-zero temperatures can cause risk to personnel, facilities and equipment. These risks include power outages, auto accidents, slips and falls, frostbite, hypothermia and building collapse.Resources: Snow blowers, shovels, scrapers, anti-slip materials, deicers, local radio stations, internet weather maps, snow removal contractors, and resources listed in the power outage procedure.Snow and Ice Storms: The Facilities & Safety Manager will make arrangements with a Snow Removal Contractor to provide parking lot snow removal and anti-slip services when precipitation falls and the air temperature is below 35 degrees. Maintenance will coordinate the application of anti-slip and snow/ice removal from sidewalks, paths, and roof drains and clearance at all emergency exits. The Facilities & Safety Manager and other company personnel will provide assistance as needed.Roof Loading: The weight of snow and ice can cause buildings to collapse. The Maintenance department and the Facilities & Safety Manager will monitor facility conditions to look for signs of potential problems. These signs include sprinkler heads protruding below ceiling tiles and excessive snow accumulation on the roof. Plant drawings and engineering consultants will be utilized to determine when steps must be taken to reduce the roof loading. Maintenance will coordinate snow removal efforts. The Facilities & Safety Manager, the Snow Removal Contractor, and other company personnel will provide assistance when needed.Plant Closings: The senior staff will determine plant closings as described in the policies and procedures manual. Any related announcements will be made via email during working hours. Employees can also call 314-531-8839 (Ext. 3334) for work status.Flood Emergency Response Plan1.0 PURPOSEThe Killark facility is within the flood plain of the Mississippi River. A flood can have devastating effects on homes, businesses, communities and the region. By defining the steps to take when a flood is imminent, business disruption and losses can be minimized. 2.0 SCOPE2.1The scope of this procedure is limited to Killark, St. Louis, MO.3.0 DEFINITIONS3.1Company Authority: The person who has the ultimate authority to activate the FERP and redirect resources and, ultimately, shut down operations if necessary. For the purposes of this plan, the business unit Vice-President, Warren Jemkins is the primary Company Authority. The back up Company Authority is the Operations Director, Mike Collins.3.2ERT: Emergency Response Team: The Facilities & Safety Manager, First Aid Responders, Hazardous Materials Operations trained personnel, Maintenance department, and the supervisors/employees with flood related responsibilities.3.3ERT Leader: The Facilities & Safety Manager, Steve Kerr is the ERT Leader and will coordinate pre-flood procedures and post flood response. The back up for the ERT Leader is the Maintenance Lead, Mike Button.3.4FERP: Flood Emergency Response Plan: Advanced plan that details policies and procedures to follow before, during and after a flood.3.5 Management Team: Senior managers of Accounting, Sales & Marketing, Operations, Engineering and MIS. 4.0 PROCEDURESPersonnel Safety is the number one priority throughout these procedures. These procedures may only be done when they can be done safely.4.1When Flooding is Imminent4.1.1The ERT Leader will monitor weather forecasts, road closures, and stream/river cresting forecasts via local radio, television, NOAA weather radio, local emergency responder radio and the internet. 4.1.2The Company Authority, based on information from the Management Team and the ERT Leader, will determine if and when to evacuate the facility.4.1.3The ERT Leader will make the following contacts:ContactOfficeCellFM Global: Randi Genuchi636-332-1045Hubbell Corporate Risk Management Director: Gene Surrett475-882-4366203-915-54794.1.4Ground floor personnel will coordinate with the MIS department to relocate computers and multi-function printers to the second floor.4.1.5MIS personnel will coordinate the shut down and relocation of servers and other network hardware. Additional MIS procedures are detailed in Disaster Recovery Plan DR TEST RESULTS REPORT-SHELTON-2-27-11.doc.4.1.6Ground floor managers will coordinate the relocation of critical documents to the second and third floors.4.1.7Maintenance personnel will coordinate the shut down of all utilities including electric circuits, emergency generator, phone lines and gas lines. Maintenance will also monitor all roof and storm drains to insure that they remain clear.4.1.8Plant personnel will cover all equipment susceptible to water damage with plastic or tarps.4.1.9The HR Manager, Lori Sharp, will maintain employee communications through local radio stations.4.2After the Flood4.2.1The Management Team, ERT Leader and Maintenance department will coordinate a complete inspection of all flooded areas to determine any structural damage, cleaning/disinfecting requirements and to determine if utilities can be restored. No additional personnel will be allowed into the building until an area is determined to be safe from hazards. Where necessary outside services will be contracted to expedite the flood area clean up and restoration (resources are listed in the Business Continuation Plan). 4.2.2Local emergency response agencies such as the St. Louis Fire Department and their mutual aid companies will also provide assistance when requested.4.2.3The ERT Leader will report the inspection findings to the contacts listed in 4.1.3.4.2.4Plant managers/supervisors will coordinate removal of covers from equipment. Any equipment that may have gotten wet will be kept out of service until inspected by Maintenance or Q/A Engineering.4.2.5Maintenance and Plant personnel will coordinate water/moisture removal. Dehumidifiers and wet vacuums will be used where feasible.4.2.6Ground floor personnel and MIS will coordinate the return of equipment and documents.5.0 TRAINING & PLAN REVIEW5.1Training in the form of a tabletop exercise will be conducted annually. Participants will include the Company Authority, ERT Leader, senior management, maintenance and plant managers.5.2The FERP will be reviewed annually during training and when ever the plan is implemented. If power is out, system will not function properly.Tyco contact. The computer room fire suppression system will signal Tyco Integrated Security when power is disrupted.Visit facility and check functionality of equipment, lighting, etc.Steps to Take During Outage:Open Emergency Communications Room.Contact Ameren at 314-342-1000 to report outage and to determine when restoration of service is expected. Contact Crisis Management Team.Maintenance: Switch off high demand equipment including- HVAC Equipment and Air Compressors.Crisis Management Team Steps:Meet at facility conference room.Use plant radios for facility communications and flashlights for lighting.Contact department Managers & Supervisors as appropriate using Cell Phones.Determine if next work shift can begin as scheduled.Contact local radio stations to schedule announcements if appropriate.Contact NU-WAY Rents at 314-544-1214 to determine availability and feasibility of renting a generator to power specific heating equipment. Continue communications with Ameren at 314-342-1000 to monitor restoration status. Steps to Take After Power is Restored:Power up equipment systematically.Verify equipment is functioning properly.Contact management and employees as necessary.Review plan for changes.* See Power Outage Plan – Regular Business Hours for additional steps to be taken.Power Outage Plan - Regular Business HoursBackground: Power outages constitute a significant risk due to the wide range of potential causes. Power outages can occur due to natural disasters such as the high winds associated with hurricanes and tornadoes, thunder storms, ice storms, earthquakes and floods, and outages can also occur from manmade disasters such as fires, car accidents, plant equipment failure and utility failure. Therefore, preparations must be made to limit the impact of a power outage. The following procedures will be used as a guide. However; specific circumstances may dictate additional actions.Resources: Emergency generator, battery powered emergency lighting, emergency exit lighting, flash lights, paging system, phone system and plant radio system.Employee Procedures: A facility representative will contact the utility company, Ameren (314-342-1000), during the first minute of an outage. The utility company should be questioned as to the cause of the outage, the geographic area of the outage and the expected duration. Any specific information the utility provides should be relayed to the senior management, Maintenance Lead, and the Facilities & Safety Manager.Plant personnel should slowly and safely relocate to the Break Rooms. Supervisors should provide additional lighting with flashlights in areas where necessary and should assist any employees with special needs. Personnel in the front office areas should relocate to areas that have sufficient lighting for their comfort. Supervisors must maintain employee accountability. To maintain emergency communications, employee phone usage must be limited and for short duration. First Aid Team members will keep their first aid kits with them to provide emergency assistance if necessary.The senior management will make additional decisions such as plant closings on a case by case basis.Facility Procedures: In order to avoid significant power surges, damage to equipment and overloading on emergency power backup, the following equipment should be switched off during a power outage by all employees- personal computers, bench & work station lighting and other bench equipment. Telephone calls should end as soon as possible to open phone lines and to reduce the load on the telephone system emergency power backup.Maintenance personnel will coordinate the shut down of all large equipment including- HVAC Equipment and Air Compressors. Appropriate breaker panels will be marked to indicate which breakers must be shut down during an outage.Mission Critical Activities: Steps should be taken to minimize customer inconvenience during an outage. Telephone lines will be maintained by a UPS located in the telephone room. Power Restoration: Every effort will be made to facilitate power restoration as quickly as possible. Where appropriate, contractors will be utilized to repair plant equipment involved in the outage. Communication with the utility company will be maintained during extended outages which are the result of loss of service. A facility representative will contact the utility at least hourly to check the status of repairs and the estimated restoration of power.When power is restored after a five-minute or longer outage, employees should wait two minutes to return to their work areas to verify that the restoration is stable. Maintenance will restore power to all larger equipment.
