Instructions ― Request for Leave, Absence Report

Employees are to complete a line for each period of time leave is requested. At the end of the month the manager compares the employee’s MSC 46 form with the time entered into the Online Daily Time Capture System, signs the form and locks the time.

• Leave time request start ― Employee indicates the date and time the leave is to start.

• Leave time request end ― Employee indicates the date and time the leave is to end.

• Date/time of request ― Employee indicates the date and time the leave is requested.

• Hours requested ― Employee indicates the amount of time requested for leave.

• Reason for leave ― Employee indicates the type of leave requested (e.g., vacation, sick, personal business).

• Employee Initials ― Employee initials for each request.

• Manager approved/denied ― Manager as applicable checks the appropriate box, initials the request and returns the document to the employee. If denied, the manager must also indicate the reason in the section at the bottom of the form.

• Leave code ― Employee indicates the leave code that will be used for time reporting purposes.

• Hours used ― After taking the leave time, the employee indicates the actual number of hours used.
