|SUBJECT: |CARE Change Control Information – June 26, 2015 |

|Purpose: |To provide information about the upcoming CARE Change Control Release |

|Background: |This CARE Change Control Release will occur on Friday, June 26, 2015 starting at 5:35 pm. CARE users will receive|

| |the new version when they log into CARE any time after 8:00pm on Friday. |

| | |

| |No action is required by users for change to occur. CM/SSS/CRMs should check their cases in and close CARE before|

| |leaving Friday June 26, 2015. |

| |CARE Change Control Release: Summary of Changes |

| | |

| |ProviderOne client ID has been added to the footer of the CARE Service Summary |

| |Provider search in the Authorization screen replaced ‘Address 2’ with ‘City.’ |

|ACTION: |PLEASE NOTE: |

| |All pending assessments DO NOT have to be placed in current status by June 26, 2015 at 5:35 pm |

| |The CARE Conversion will occur on June 26, 2015 at 5:35 pm. |

| |CM must have all assessments, pending and current checked in for this conversion. |

| |All pending assessments that have all green diamonds, and have the Care Plan screen completed will convert to |

| |their new respective program when applicable. |

| |All assessments with plan periods ending 6/30/2015 must be moved to current by 6/30/2015 based on existing policy.|

| | |

| |If an annual or significant change assessment is left in pending status after June 26, 2015 and the assessment |

| |results in an increase or no change in hours: |

| |Keep the converted program in the “client is eligible for” dropdown in the Care Plan screen in CARE that will be |

| |in effect on July 1, 2015. |

| |Select the appropriate CFC program in the PAN and enter 7/1/2015 as the effective date. |

| | |

| |If an annual or significant change assessment is left in pending status after June 26, 2015 and the assessment |

| |results in a decrease or termination: |

| |The client must receive a PAN based on “10 to the end” that will not be effective until August. For a reduction in|

| |services, keep the new converted CFC related program in the “client is eligible for” dropdown in the Care Plan |

| |screen in CARE that will be in effect on July 1, 2015. |

| |Select the appropriate CFC program in the PAN and use the system generated effective date. (Remember, that if a |

| |client’s program is going to change, they will have already received a notice letter notifying them of the program|

| |change. The PAN will only need to notify them of the reduction services.) |

| | |

| |Please see the CARE Change Release notes (attached) for more details in addition to a detailed description of all |

| |of the changes related to this release. |

| | |

| |CARE Practice will be released with the updates on Friday, June 19, 2015. |
