HOW T O ENROLL

ENROLLMENT PROCEDURE ? All full--(me employees and their legal dependents are eligible to enroll in the same membership

(er. ? To enroll, renew (regardless of which club(s) you'd like to use), change membership level or cancel

your membership, you may either e--mail Corporate Account Execu(ve, Katherine.Marion@, or call 212.964.6688 to get set up.

For first (me users, to visit a loca(on, please call 212.964.6688 to schedule an appointment. ? You will need your Ambrose employee ID number (the same number used to log onto ).

ENROLLMENT/CANCELLATION DEADLINE ? All enrollments, changes and cancella(ons can be done between the 2nd and 25th of the month

to be effec(ve the 1st of the following month. ? For example, to cancel your membership for the month of June, you must request the cancella(on

by May 25.

PAYROLL DEDUCTIONS ? Monthly membership dues will be deducted on the mid--month payroll for that month's

membership dues.

If you are paid at month--end only, the deduc(on will take place on that payroll. ? For example, if you are paying for the month of April, your membership dues will be taken from your paycheck on April 15.

DESTINATION ACCESS MEMBERSHIP

$225/month : Access to all Equinox Fitness Clubs (U.S. & Interna(onal).

ALL ACCESS MEMBERSHIP

$178/month : Access to all clubs across the U.S. excluding the "Des(na(on" clubs (Prin(ng House (NYC), West LA (CA), and Irvine (CA)).

CORPORATE ACCESS MEMBERSHIP

$155/month : Access to all clubs across the U.S. excluding the flagships in Manhaaan (Rockefeller Center, Greenwich Avenue, Columbus Circle, 76th Street @ Amsterdam, Lexington @ 63rd Street, & 74th Street @ 2nd Avenue) and "Des(na(on" clubs (Prin(ng House, West LA, & Irvine).

BOSTON ACCESS MEMBERSHIP

$137/month : Access to all Boston clubs.

CHICAGO ACCESS MEMBERSHIP

$132/month : Access to all Chicago clubs.
