School Counseling Disclosure Statement

I have developed this statement to inform you about our counseling program and my role in your educational growth. It is my desire that the information given proves to be beneficial to you and meet your specific needs. If there are any questions or concerns regarding our counseling program, please feel free to stop by my office at any time.

Counseling Approaches and Background:

Mrs. Tiffany Gibson-Pitts, M.Ed., Ed.S.- 9th & 10th School Counselor. This is my fourth year as a school counselor. My approach is based on helping students with personal and family issues that may interfere with their ability to learn. Through individual, group, and classroom guidance, I will help students deal with these issues, and then help students, teachers, and families implement plans to resolve these problems. My goal, as a school counselor, is to provide an ideal environment for achieving academically.

Appointments: My classroom guidance and small group interventions schedule will be posted on my door for your convenience. The scheduling of appointments is unnecessary, but is available if you so desire. I am also available for after school appointments each Thursday until 6:00pm. After school appointments must be scheduled by 3:00pm each Wednesday. If I am not in my office, I can be reached via email at gibsontd@ or at 756-4105 ext.306.

Confidentiality: Information shared between a student and a counselor is confidential. Confidential means that the information shared between you and your counselor will not be shared with others except under specified circumstances. The exception, or conditions are listed below:

• If your counselor believes that you are in danger, or that you present a danger to others

• If a judge order your counselor to tell others

• If you report behavior in violation of school policy that your counselor is required to tell the school administration (identified in your student code of conduct handbook)

• If your counselor need advise or feels that professional assistance can no longer be provided to you. Appropriate referrals will be made

• If you ask your counselor to tell someone else

Benefits: You may experience closure to problems that were affecting you from becoming academically successful. You may feel better about yourself because you have talked to someone who understands what you are experiencing and do not judge you on past mistakes.

Risks: You may feel uncomfortable when exploring personal issues.
