Sunrise Response to Intervention (RTI) Checklist

Walter C. Young Response to Intervention (RTI) Checklist

TIER 1

_____ Identify the students in your class who are struggling in one or more areas (academic or behavioral) compared to the rest of your students.

General Ed. Accommodations Class-wide Reinforcement

Classroom Management System Class-wide education computer-based

TIER 2

_____ If a student has not made progress after at least 4-6 weeks of Tier 1 intervention, schedule a meeting with your grade-level case manager, as well as any other colleagues with whom you wish to consult. At this meeting you will …….

1. Develop a Tier 2 intervention plan. Tier 2 interventions should provide the student with additional instruction, practice time, or behavioral/emotional support than was being received in Tier 1. Interventions may include the following:

Peer tutoring

Struggling Reader Chart

Small Group with the Reading Spec.

Weekly/Daily progress monitoring of academics

Family/ Individual Counseling

Behavior Modification Plan

Schedule Change

Credit Recovery Program

READING INTERVENTIONS?

2. Document your Tier 2 Intervention Plan on the Tier 2 Form. (Your case manager will have forms.) Teacher will give a copy to the case manager.

3. Share plan with parents.

4. Monitor progress in Tier 2 for 4-6 weeks, taking data points.

Note: Some students may begin the school year receiving Tier 2 interventions through ELL, PMP, or may be continuing other Tier 2 interventions developed last year. In these cases, you will need to have a meeting with your Case Manager to document what Tier 2 interventions are currently in place for these students and determine if additional Tier 2 interventions are needed. You do not have to fill out a TIER 1 form, since the student is already in Tier 2.

Tier 3

_____ If a student has not made progress after 4-6 weeks of Tier 2 intervention, schedule a RTI/CPST meeting through your case manager. At this meeting you will…..

1. Bring the student’s Tier 1 and Tier 2 Forms, current grades, work samples, progress monitoring data, and/or behavior charts/discipline records to share with the team.

2. Collaboratively develop a plan of action. NOTE: This may include a full evaluation, additional Tier 2 interventions, and/or the creating of a Tier 3 intervention plan.

The following are examples of Tier 3 interventions. Tier 3 interventions are evidence-based, individualized, may occur daily, involve no more than 3 children to a group, and are accompanied by routine data collection for progress monitoring.

Evidence-based, individualized interventions in classroom

Groups of 3 or fewer students with reading/math teacher or ESE certified teacher daily

Individual Counseling support daily

Individual Behavior plan

Daily after school tutoring program (with progress monitoring)

Tier 2 intervention implemented daily in groups of 3 or fewer

FBA/PBIP

Referrals to Family Counseling Center, Memorial Health, Chrysalis, Henderson Mental Health and other community mental health agencies.

3. If the team decides to create a Tier 3 plan, this will be documented on the Tier 3 Form and signed by team members.

4. Parent is given a copy of plan.

_____ Begin implementing the Tier 3 intervention developed by the RTI/CPST, and continue doing so for 4-6 weeks.

_____ For academic interventions: Each week, administer one of the Progress monitoring probes (as suggested in the Struggling chart) and graph results.

_____ For behavioral interventions: Use behavior charts to keep track of how often the target behavior occurs. Count up totals and graph daily/weekly.

_____ A follow-up RTI/CPST meeting will be scheduled after this 6-week period. Bring the completed Tier 3 plan, plus the student’s current grades, work samples, progress monitoring data, discipline records, and any other pertinent information to this meeting. The team will review the student’s progress and determine a plan of action. This may include dropping back down to a Tier 2 intervention, remaining in Tier 3, or referring for a psycho educational evaluation.

• If the student remains in Tier 3, the classroom teacher will remain RTI Case Manager and will be responsible for placing all RTI data into the student record file at the end of the year.

* If the student is referred to Psych Services, the ESE Specialist will be

responsible for the RTI data and placing it into the student record after the eligibility staffing.
