Dear Medical Use of Marijuana Program:

This letter is in response to your Request for Information for Giving Tree Health Center, Inc. 's ("GTHC") Siting Profile (Application 1 of 3). Answers to the questions are below.

1. Please see an updated table in Section B that clarifies the addresses of the dispensing, cultivation, and processing will occur. As you will see in the updated table, the dispensing will occur at 867 Pleasant Street while the cultivating and processing will occur at 847 Pleasant Street. Both of these addresses are on the same property and included in the same Intent to Lease document.

2. Fall River has not adopted local siting requirements and bylaws for registered marijuana dispensaries. The City Council, however, has approved the location of the site. Below is a list of uses within 500 feet of the proposed facilities: ? Retail ? Restaurant Establishments ? Medical Facilities ? Financial Institutions ? Automotive Service Providers ? Cosmetology ? Event Space

4. Background check authorization forms were resubmitted to the Department for Brian Bairos and Brian Mclsaac on July 23, 2018. As previously explained with that submission, Alfredo Pereira is no longer a member of the Board. Additionally, Adam Demara! is no longer CFO and has been replaced. Updated application materials have been submitted to the Department that reflect this change.
