All applicants to the Saline County Career Center Practical Nursing Program must submit and pass a background screening through the Missouri Department of Health & Senior Services.
Saline County Career Center will process the background check.
To do this we need you to please complete the Missouri Department of Health and Senior Services Family Care Safety Registry Worker Registration form.
The FCSR form needs to be sent to Saline County Career Center. SCCC's Application Fee covers the cost of this background screening.

Complete as follows:
Registration Type: Select VOLUNTARY Social Security Number: Provide number and copy of social security card. Personal and Contact Information: List current name, and any other names used, and current address. Employer Associated with this Registration: Complete column on right and select No Employer and select Volunteer. Sign and date. Return completed form to: SCCC Nursing Program 900 West Vest Marshall MO 65340
