JOB ANNOUNCEMENT

ADULT FAMILY PRACTICE/INTERNAL MEDICINE PHYSICIAN OPPORTUNITY

The Family Practice/Internal Medicine Physician at Mission Neighborhood Health Center will be primarily responsible for providing services to Adult Medicine patients. The Adult Medicine Provider would be part of a team charged primarily with the implementation of the plan to fully utilize the power of the recently implemented EHR and to achieve eventual Patient Centered Medical Home recognition. The provider would need to support and lead some process improvements in access, population based patient management, patient care management, patient/family self care support, referral tracking, and clinical quality performance evaluations. Evening or weekend hours may be needed to support expanded access in the future.We will support loan repayment at a federal and state level as needed.

QUALIFICATIONS

• Licensed in the state of California and current DEA registration

• Board certified/Eligible or residency trained in Family Practice, Internal Medicine or Osteopathy

SPECIALIZED SKILLS, KNOWLEDGE, AND/OR EXPERIENCE

• Bilingual in Spanish and English required
