Volunteer Clearance Information Sheet

1. ACT 114 – FBI Clearance - Register with Identogo through their website: . Use

service code 1KG6XN. Take your receipt and a valid photo ID to an Identogo office (office locations are listed on their website or call them toll free at 844-321-2101. Once your prints have been taken, please return a copy of your receipt containing your UE-ID number to: Cindy Donovan at cdonovan@avonworth.k12.pa.us or the Avonworth School District Office, 258 Josephs Lane, Pgh., PA 15237.

NOTE: Volunteers may still be exempt from obtaining FBI criminal background checks if he/she has continuously resided in Pennsylvania for 10 years before beginning serving as a volunteer. Volunteers may also be exempt from obtaining FBI criminal background checks if they have not resided in Pennsylvania for 10 years but have received an FBI criminal background check elsewhere during his/her PA residency and has provided it to the school district. The volunteer must still sign an affidavit swearing or affirming that they are not disqualified from volunteering or have been convicted of a criminal offense that would prohibit him/her from volunteering.

2. ACT 34 – Pennsylvania Criminal History Check - Please use the website for this clearance.

Go to “New Record” and follow the instructions through to the Certification

Page. Once the Search Results Table appears, click on the Control Number. (Write down the Control Number for future reference.) The Record Check Details page is opened. Click on the words “Certification Page” to access your official Clearance certificate and print the certificate. (It doesn’t look like a button to click on but if you hover over the words “Certification Page” you’ll be able to print the certificate.) The Record Check Details page is only a receipt and not acceptable as a ‘clearance.’

3. ACT 151 – Pennsylvania Child Abuse Clearance - Please use the website for this clearance.

To apply for your Child Abuse clearance online go to pass.state.pa.us/cwis/public/home See “Welcome to the Child Welfare Portal” and click on “Create Individual Account.” NOTE: On the “Create Keystone ID: Profile Information page, you’ll need to create your own Keystone ID. They will email you a temporary password. You will need to go back into the site and enter that temporary password and follow the instructions. Call the Compass help line at 1877-371-5422 with questions or for assistance.

4. Act 126 of 2014, Mandated Child Abuse Reporter, Pennsylvania State Law In accordance with Act 31 of

2014, as a school employee that interacts with students, you are required by the state to be a mandated child abuse reporter.  Being a mandated reported requires you to report any form of child abuse that you have a reasonable cause to suspect.   For more information on mandated reporting please visit this link:

You may complete the training online at this link:  .  This is offered at no cost through the University of Pittsburgh.  Upon completing this training, be sure to print your certificate and provide a copy to Cindy Donovan in the District Administration Office.
