Instructions: If you are a parent of a dependent student and have been approved for a Federal Direct Parent PLUS Loan, complete ALL items on this form. Please submit this completed form along with a copy of the borrower's Driver's License or State Identification Card to the Office of Scholarships and Financial Aid.

To be eligible to borrow a Federal Parent Plus Loan, the borrower must: ? Be the biological parent, legally adoptive parent or the step-parent (who reported information on the FAFSA) ? Be a US Citizen or be a Permanent Resident (visa status for noncitizens) ? Not be in an overpayment on any federal student aid ? Not be in default on any federal student loans ?
