Provider Portal Overview

The provider portal gives medical providers access to online Explanation of Benefits (EOB) and HERO reports through data provided daily by the Financial Management System Austin Information Technology Center (FMS-AITC).

The portal presents Dialysis claim EOB information in a searchable format and provides user profile management.

This document explains the provider portal functionality for Financial Services Center (FSC) help desk users.

Initially, you request system access, create a new user account, complete registration, and log in to the system. If you forget your password in the future, the system provides a mechanism for resetting your password.

Request Access

To gain access to the system, VA personnel must complete and digitally sign a 9957 form. Forward the form to your supervisor who will also digitally sign and submit it to the FSC via e-mail to fscsecurityrequests@. FSC must have your completed 9957 on file in order to grant access to the system. Only electronic copies are accepted. An electronic copy of the 9957 form is located at .

1. Mark the privacy training box at the top of the form.

2. Request Windows access.

3. Ensure the change type is Modify Existing User.

4. Complete all of Section 2.

5. In Section 3, specify that you are adding access to the CHAMPVA Provider Portal.

6. Once completed, e-mail the electronically signed form to FSC Security Requests at fscsecurityrequests@.

7. A bulk 9957 can be submitted for large groups of users with an attachment detailing individual users.

Create A New Account

1. From Internet Explorer 7 or later, go to .

8. In the lower right area of the screen, click on Click here to create a new account. The system displays a Provider Portal registration request screen.

9. Complete all of the fields on the registration pop-up window. In the First Name and Last Name fields, enter the name of the person who will be the primary user of the portal.

10. In the Email field, enter the e-mail address of the primary user.

11. In the Password field, type a password you will use for system access.

Your password must be at least 12 characters in length and must contain:

• 1 upper case character

• 1 lower case character

• 1 number

• 1 special character (! @ # $ %, etc.)

12. In the Confirm Password field, re-enter the same password.

13. If you ever forget your password, the system provides a way you can recover your password by asking you two security questions and comparing that to the answers you provide. Your correct answers to these two questions help confirm your identity and keep your password safe.

Think of two questions that only you can know the correct answer to. Then, in the First Password Question text box, type the first question.

14. In the First Password Answer text box, type the answer to that question.

15. In the Second Password Question text box, type the second security question.

16. In the Second Password Answer text box, type the answer to that question.

17. Click on Register. The system displays a confirmation message indicating your registration was successful and that a confirmation e-mail has been sent to the e-mail address you entered.

18. Click on OK.

Complete Registration

1. Start to your e-mail reader and open the e-mail titled Complete your registration of Staff Portal.

19. Click on the URL included in the e-mail. The system redisplays the Provider Portal Registration screen with the message Your registration is complete.
