Medical Insurance How to Find Your Doctor's Medical Group #
In order to enroll in the BCBSIL Blue Advantage HMO, you will need to provide your doctor's Medical Group/IPA #. If you are already a member of the BCBSIL Blue Advantage HMO, this number can be found on your medical insurance card as highlighted on this sample.
How to Find an In-Network Doctor
To locate an In-Network provider, enter the Blue Cross website by clicking here. Once in the site, answer the questions under "Find an In-Network Provider" by completing the questions as outlined on the following page.

Select Blue Advantage HMO? [ADV] for a listing of HMO providers. Select Participating Provider Organization [PPO] for a listing of PPO providers.

If you have a doctor, and want to know if they are in the plan, enter the doctor's last name in the first box and click "Find a Doctor or Hospital." By clicking on "More Search Options" you can narrow your search by the categories listed.

Based on the search criteria, a list of doctors will appear.
If you are looking for a particular doctor, and he/she does not appear, check the spelling, location or any other search information you entered and try again. If the doctor still does not appear in the list, they may not be in the network you are searching. In these situations, it is best to call their office and ask if they are in either the Blue Advantage HMO or the PPO network, depending on the plan you enrolled in.

Once you have decided on the doctor you wish to see, click on their name to locate their Medical Group / IPA # and to confirm whether they are accepting new patients.
For Blue Advantage HMO participants, scroll down until you see Medical Group / IPA #. In this case, it is 082. Jot this number down so that you can enter it into myACCESS when enrolling in the Blue Advantage HMO plan.

How to Find an In-Network Dentist
Delta Dental PPO and DeltaCare HMO Provider Finder
To locate a Delta Dental network dentist, enter the Delta website by clicking here.
To find a dentist close to your home or office, enter the zip code and click on "Search." If you are looking for a dentist in the DeltaCare DMO plan, be sure to choose DeltaCare as your network.
