Enter you UserID, Password and the Security Code. Click on the Login button. You will then be redirected to the Form Submission page.At the Local Government Electronic Filing System log in site, I entered my UserID and Password to log in, but it says my login information is incorrect. What should I do?If you have forgotten your UserID, please do not create another one and contact our division for assistance. If you have forgotten your password, enter your UserID and click the “Forgot Password” link. Enter your UserID and the email address you used when creating the account. A new temporary password will be emailed to you. If you have login problems after receiving a new password, you may be entering an incorrect UserID. Please call our division at 517-335-7469 for assistance.I file for our primary local unit of government, but also need to file for an authority in our local unit. I do not see the authority in my drop-list. Do I need another User ID?No. By changing your User profile, you can file for another municipality within the primary local unit. To change your User profile, click the User Profile link. Complete all fields marked required (*), certify at the bottom of the page, then click on submit. Remember to change back to your primary local unit after you have submitted your form. Be sure to check the “I hereby certify….” box before clicking on the Submit button.I made my changes in my User Profile and clicked save. Why didn’t anything happen?Make sure that you’ve completed all required fields (*), including checking the certification box at the bottom of the page. I selected my form and fiscal year, then closed the form prior to completing it. Why does the fiscal year now display as yyyy-I in the drop-list?An “-I” indicates the form was opened but interrupted (closed) and not completed. The interrupted form can still be selected and completed.After logging in and selecting my municipality type, why am I being asked “Audit Filed Under?” This question is asked to all non-primary units of government (authorities, hospital finance and libraries), so it can be determined if you file your own audit or if your municipality is included in the audit report of the primary local unit. Select your municipality name from the drop-list ONLY IF you file your own audit; otherwise, select the name of the municipality whose audit you are included in.I answered a question incorrectly on my form but have not yet submitted. It appears I cannot go back to correct my error. What should I do?If the error was made on a Short or Long Term Prior Approval Application or Exemption form, go ahead and submit your form. Send an email to Treas-DataSolutions@ informing us you need to make modifications. We will push the pre-populated form back so you can make any necessary correction.If the error was made on the Auditing Procedure Report (APR), unfortunately you will need to delete the report and start over. The form can be deleted from the form selection page by selecting the checkbox in the ‘delete’ column for the APR and clicking the ‘delete’ link. I had my form open for quite a while, but I was actively working in it. When I finally hit save (or submit), I was returned to the login page. What happened, and did I lose my information?Unfortunately, unless you periodically hit the save button while working on your form for a long period of time, the system will time-out without warning. Your work will not be saved.Please note the Qualifying Statement form must be completed at the time it is opened as it cannot be saved and retain the information entered.How do I submit a revised audit report? A request must be sent to Treas-DataSolutions@ by the Chief Administrative Officer (or designee) of the Local Unit indicating the correction(s) that are necessary. Upon receipt of this request, we will clear the prior submission and inform the CPA the audit report can be refiled.What are the consequences of filing a revised audit report?The original filing date of the audit will be changed to reflect receipt of the revised audit, thereby potentially making the filing status late. Late audits affect the qualifying status of the local unit or a municipality that is part of the local unit’s audit. How do I request an extension to file my audit report?The Chief Administrative Officer (or designee) must send an emailed request to LAFD_Audits@ explaining why the extension is needed. The request will be reviewed and a response provided by the Manager of the Audit and Accounting Section. How do I request an extension to file my Annual Financial Report (F-65)?The Chief Administrative Officer (or designee) must send an emailed request to LAFD_Audits@ explaining why the extension is needed and attaching an unadjusted year-end trial balance report. The request will be reviewed and a response provided by the Manager of the Audit Section. I’ve completed my F-65 form. How do I submit it?Once you have completed filling out your form, select the “Next: Attach” tab to submit. How do I fix the errors I am receiving when I attempt to submit my F-65 form?A member of our audit staff would be happy assist you with any prevent submissions you may be experiencing. Please contact our office at 517-335-7469 to speak with an audit staff member.How do I file a Corrective Action Plan?If you do not have a UserID and password, you will have to go online to request access by clicking on the “Request Access” link. If you already have a UserID and password, log into our website at . Click the File Online Reports tab. Enter your User ID, Password and the Security Code and click on the Login button. Select from the Form drop list Corrective Action Plan for Audit, select the Fiscal Year and click the Create Form button Navigate to your desktop where you saved your Corrective Action Plan (must be PDF), click submit. Note that you will only be able to attach one document. If you are unsure of what to put in your Corrective Action Plan, please ask to speak to one of our auditors at 517-335-7469.I submitted a Qualifying Statement form. How soon should I expect to find the results on the public website? If the audit report has been filed prior to submitting the Qualifying Statement, the results of the Qualifying Statement will be posted within one business day. However, if the audit report has not been filed, no results will be posted until it has been filed or the deadline for filing has passed. You will not receive notification a determination has been made other than by the posting on the public website.My F65 form is not saving and I cannot submit it.Only one User can be logged into the form at a time. Be sure only one computer screen is opened to the electronic filing site when attempting to Save and/or submit your form.I’m a new User attempting to sign up to file my forms. When I click on the “Request Access New User” link and select my municipality, the Organizational Mailing Address is greyed out and I cannot type in my information. The municipality’s address is not in our system. Please contact the Division at 517-335-7469 as you cannot proceed until we receive this information. ................
