Ambetter from Sunshine Health (Ambetter) makes it easier for you to get the care you deserve--from the doctor you trust. We partner with Primary Care Providers (PCPs), specialists and other medical providers.

In this book, you'll find a list of all of the providers in our network. If you choose to visit a provider that is not part of our network, your costs for receiving care will be affected. Remember that you might need a referral from your PCP in order to get care from certain providers or specialists.

The Provider Directory has a list of all the providers in our network.

We keep our list of providers updated and produce this directory regularly. For more information about a provider in this directory, visit Ambetter. or call Member Services at 1-877-687-1169 (Relay FL 1-800-955-8770 ).

Your health. Your choice.

Your PCP is the main doctor you will see for most of your medical care. This includes your check-ups, sick visits and other basic health needs. When you need care, all you have to do is call your PCP for an appointment.

All Ambetter from Sunshine health members will need to select a PCP. Use this directory, visit Ambetter. or call Member Services at 1-877-687-1169 (Relay FL 1-800-955-8770 ).

Make sure to bring your Ambetter Member ID card when you receive medical care or fill prescriptions.

We believe that having a good relationship with your PCP makes it easier for you to stay healthy. Be sure to choose someone you are comfortable with as your PCP. We recommend that everyone in your family have a PCP.

This directory also includes vision and dental providers who are part of the Ambetter from Sunshine Health network. If vision or dental is part of your plan, you will be covered when visiting any of the providers listed in the directory. If you did not buy a vision or dental plan, Ambetter won't cover visits to the providers listed.

We understand that you might need some extra help. If you use a wheelchair, walker or another aid, let your PCP's office know when you make your appointment. Ambetter can always help you with talking to your doctor about your special needs. Just call Member Services at 1-877-687-1169 (Relay FL 1-800-955-8770). We can help you access your PCP's office, and someone will be ready to help you when you get there.

Stay in charge.

Call your PCP if you have an illness or injury that needs attention right away. If your PCP isn't available, call our 24/7 Nurse Advice Line at 1-877-687-1169 (Relay FL 1-800-955-8770 ).

For serious emergencies, call 911 or visit the Emergency Room (ER). Call your PCP or our 24/7 Nurse Advice Line if you're not sure if you should go to the ER.

Some examples of medical emergencies are: - Broken bones. - Gun or knife wounds. - Bleeding that won't stop. - Labor and/or bleeding during pregnancy. - Severe chest pain or heart attack. - Drug overdose. - Poisoning. - Bad burns. - Shock (symptoms: thirst, dizziness, pale skin). - Convulsions or seizures. - Trouble breathing. - Sudden inability to see, move or speak.

You can use any hospital to receive emergency services.
