| | |bad cords, missing or broken parts, cracks and/or significant|

| | |wear, etc. will be noted and tool will be locked out and |

| | |tagged out and removed from the job. All employees are |

| | |trained in the use of tools of our trade. |

| | |proper PPE and by trained personnel with approved hotwork |

| | |permits. All cylinders will be secured and tools will be |

|Procedures |

|Employee Orientation |Every employee before working on jobsite will have a safety orientation covering all rules and regulations of |

| |the jobs. They will be given all safety information including the Site Specific Safety Plan and the JHA. |

| |Following they will sign the safety orientation log stating they have been trained in the safety policy and job|

| |hazards |

|Lock-Out/Tag-Out |Identify all systems affected by the shutdown. Close every energy source feeding the system and disconnect the|

| |direct source of energy. All locks and tags are only to be removed by the employee that put them on. Removing|

| |a lock or ignoring a tag is cause for dismissal from the company. |

|PPE |Required PPE at all times: Hard hat, gloves, safety glasses, and boots. All other necessary personal |

| |protective equipment must be worn if necessary for the task. Any willful violation of HPS’ PPE rules are cause|

| |for dismissal from jobsite. |

|Trenching and Excavating |All trenching and excavating greater that 4.5 feet deep will be monitored by a certified competent person. |

| |Only operators trained and experienced will operate any piece of excavating or trenching equipment. See Pages |

| |47-73 of HPS Safety Manual |

|Hot Work |All hot work will follow state and federal regulations. There must be a fire watch at all times when |

| |performing hotwork. All compressed gas cylinders for welding and cutting must be properly secured and properly|

| |stored during and at the end of each work day. Hot Work permits are good for one day only and will be issued |

| |by an Authority representative. Also see section VII. A-D on page 75 of HPS safety manual. A minimum of one |

| |2A10bc fire extinguisher shall be at location of hot work and all employees shall be aware of its location. |

|Ladder/Scaffold Safety |All ladders and scaffolding will be inspected prior to each use everyday. All parts will be checked for wear |

| |and structural failure. Any defective ladders or scaffolding must be Locked out and tagged out, removed from |

| |the jobsite immediately and discarded. All workers are trained in ladder and scaffolding safety before use. |

|Fall Protection |Any work in an area or platform more than 5 feet off the ground requires fall protection. All employees must |

| |be trained in fall protection prior to using any harnesses, lanyards, nets, or lifelines. Fall protection must|

| |be inspected prior to every use and locked out/tagged out if found to be defective in and way and immediately |

| |removed from the jobsite. |

|Drug and Alcohol Free Workplace |HPS has a zero tolerance for drugs and alcohol in the workplace. Any employee suspected of being under the |

| |influence on the job will be drug screened and tested for alcohol. Any employee found to be under the |

| |influence will subsequently be terminated. |

|Jobsite Cleanup |Loose dirt and debris will be controlled by a combination of a sweeper and hand broom sweeping. Any loose |

| |debris will be contained and covered in proper containers to prevent any debris hazards. Also see Jobsite |

| |Cleanup in Project tasks and Hazards. |

|Hazard Communication Plan |All chemicals must be properly labeled and stored in a safe place. Supervisors will ensure all chemicals on |

| |the jobsite have proper labels before distribution. Employees must follow all rules of handling as described |

| |in the MSDS for that chemical. All employees will be trained before working on site. Also See Pages 90-112 of |

| |HPS safety manual. |

|Confined Space |All confined spaces may only be entered by employees training in confined space. There will be a trained |

| |attendant at all times and the confined space permit must be signed by the supervisor. All necessary |

| |ventilation equipment must be used and calibrated gas detectors must be used at all times. All spaces must be |

| |properly tested and data recorded prior to entry. Any employee entering a confined space without proper |

| |training or equipment is subject to dismissal from the jobsite. Also see pages 112-145 of HPS safety manual. |

|Code of Safe Work Practices |All employees must follow all rules set forth by HPS Mechanical. Safety training, housekeeping and rule |

| |compliance are necessary to maintain a safe work environment. Any employee not following these rules will be |

| |subject to disiniplary action.See pages 3-6 of HPS safety manual or postings in job trailer or sea train. |

|Weekly Tailgate Meeting |Weekly tailgate meetings shall be required and be documented with an attendance sheet. The attendance sheet |

| |shall be saved on the job file or sent to the PM or Safety Manager to file. The subject of the meeting will be |

| |specified and should be relevant to work being performed on the job site. Repeating relevant subjects is |

| |encouraged rather as subject matter that does not apply to the work on site.The HPS Safety Manager can supply |

| |meeting booklets as needed. |

|Heat Illness Prevention |All employees shall be trained in Heat Illness Prevention before working on site. All training shall be |

| |recorded on a sign in sheet. A copy shall be kept on site and sent to the HPS Safety Director. |

| |A copy of the HPS Heat Illness Prevention Procedures and Policy shall be readily available on site. |

| |The jobsite supervisor will be responsible for training and implementing the HPS Heat Illness Prevention |

| |Program. |

| |The Supervisor shall monitor weather forecast before each work period. |

| |On high heat(80 degrees and up) days or high humidity the supervisor shall have a short tailgate meeting to |

| |alert employees and to encourage water frequently. |

| |Water containers shall be kept as close to workers as possible and water level monitored. The amount of water |

| |shall meet or exceed Cal Osha requirements. |

| |Shade areas are required for a minimum area for 25% of workers when temperature exceeds 85 degrees and should |

| |be as close as possible but not more than 100 yards. Shade shall also be available for temperatures under 85 |

| |degrees. |

| |On Extra High heat days(over 95 degrees) the supervisor shall remind & observe workers to ensure adequate |

| |amounts water intake. |

| |Observe workers closely and frequently for alertness and signs or symptoms of heat illness. |

| |Ensure all workers shall have cell phones, radios or other means of communication and ensure workers do not |

| |work alone. |

| |The supervisor shall closely observe new employees to the site for the first week. |

| |If the employee is new to the company he should be closely observed for the first two weeks. |

|Training/Inspections |The HPS Safety Director will train or provide training required to implement all work in safe manner. |

| |All employees shall notify their supervisor immediately when an unsafe condition is seen. |

| |Jobsite Supervisors shall inspect jobsites daily and throughout the work hours. |

| |Project Manager shall inspect jobs on each visit to the site. |

| |HPS Safety Director shall inspect each job monthly or as possible if job is not continuous throughout the |

| |month. |

| |The inspections shall be Safety orientated as well as Quality Control orientated. |

| |All unsafe conditions shall be corrected immediately and reported to the HPS Safety Director immediately. |

|Jobsite Information |Required Osha information will be posted in the jobsite office trailer, sea train or be available from the job |

| |site superintendant on request. |

| |Emergency contact numbers will be included with this information. |

| |An HPS Safety Manual will be available on site. |

|Equipment & Tools |The jobsite supervisor shall verify all employees are trained to use equipment or tools they are required to |

| |use. |

| |Training on site shall be documented and a copy sent to the HPS office to be filed. |

| |All tools and equipment shall be inspected before each use and removed from service when deficiencies are |

| |found. Equipment inspection forms can be obtained from the office. |

| |

|Emergency Response |

|Accidents / Injury Response |All minor injuries, accidents or near misses are reported to on site safety supervisor and relayed to the |

| |safety committee to ensure that proper action has or will be taken. Injury report forms and investigation |

| |forms will be filled out on site in a timely manner by the site safety supervisor, injured employee, and any |

| |witnesses. In the case of serious emergencies call the appropriate number of the listed emergency response |

| |numbers or 911. The numbers can be found on this form and will be posted in the job trailer and/or sea train. |

| |All other injuries that require medical attention will require the employee to be transported to the hospital |

| |or medical center listed on the first page and posted at the jobsite. All injuries and accidents will be |

| |investigated and result in employee being drug screened. |

|Fire Protection / Prevention |Every vehicle and piece of equipment provided by HPS Mechanical, Inc. is equipped with a fire extinguisher and |

| |First Aid kit. Larger fire extinguishers will be kept on site and visible to all employees in the area. Fire |

| |safety will be discussed with crew at jobsite orientation safety meeting. Every HPS employee is trained in |

| |using fire extinguishers once a year at a weekly safety meeting. At this time every employee also reviews our |

| |Fire prevention plan. See pages 73-82 of HPS safety manual. |

|Evacuation |Should employees ever have to evacuate the site, the pre-designated meeting locations (noted upon jobsite |

|Assembly areas, egress routes |orientation safety meeting) will be utilized. The site safety representative will determine this location once|

| |an onsite evaluation can be made. |
