About Us

Christ Clinic exists to serve people who fall through the gaps and aren't able to get much-needed care without insurance. It's not uncommon for us to have patients living with extremely difficult medical conditions who have no other options. Christ Clinic began in 2000 as a ministry of the Memorial Lutheran Church in Katy, TX. Operating one night a week out of the church gymnasium, the ministry saw an increase in demand for medical services.

? In 2006, Christ Clinic moved to 5504 First Street Katy, TX 77493 and began serving patients 3 days a week. Because of continued

demand for growth and a need for independence, Christ Clinic and Memorial Lutheran Church began a separation process in 2011.

? By 2012, Christ Clinic became an independent 501c3 and operated with its own independent Board of Directors, Executive Director and by-laws.

? As of 2015, the clinic operates Monday through Thursday from 8:45 am to 7:00pm and 8:45 am to 3:00pm on Friday.

Mission Statement:

"Christ Clinic extends the healing ministry of God by offering high quality healthcare and health related services to those who are uninsured or underinsured. We serve those in need with love, compassion, respect and dignity by utilizing a team of caring volunteers and staff."

Vision Statement:

"Through our patient-centered primary care, strong provider and partner networks and diversified funding, Christ Clinic will be known for providing reliable, accessible, high quality medical services to the most vulnerable people in our community."

Christ Clinic had another successful year in 2014 helping some of the most vulnerable people in our community in accessing much needed medical care. The health care system continues to evolve across the country, and Christ Clinic remains an essential resource for meeting the needs of our community. As the country strives to change with evolving health care needs, Christ Clinic is fulfilling its vision of providing reliable, accessible, high quality medical services to those in need.

and service outcomes aligned with national standards for community clinics. While these outcomes are essential and significant, it is the personal letters and thank you messages from our patients that truly represent Christ Clinic's dedication to improving the health of our community.

We would like to thank the entire staff and our fellow Board of Directors for all the work they do in supporting the mission of Christ Clinic. The commitment of all is overwhelming and very positive. We look forward to helping the clinic continue to improve, grow and meet the demands of our growing community.

Through our patient-centered primary care, strong provider and partner network, and diversified funding, Christ Clinic achieved significant accomplishments in 2014. One of the most significant developments was the opening of the Acute Aftercare Clinic, where individuals without a primary care doctor can now easily access an evening walk-in clinic. This program amplifies the community's entire safety net system working together to ensure people are connected with and placed in a medical home of their choice. Our partners in this effort include Houston Methodist West, Memorial Hermann Katy, Spring Branch Community Health Clinic, and AccessHealth (Ft. Bend Community Health Center). This collaborative network of care broadens our ability to further address the health disparities in the community we serve.

Programs & Services ? Patient Education ? Patient education provides chronic disease management and addresses obesity, diabetes, hypertension, high cholesterol, tobacco abuse, COPD and nutrition. Education classes include: Diabetes 101, "Do Well, Be Well with Diabetes" and "Si, Yo Puedo Controlar Mi Daibetes!" (Hispanic Culturally Sensitive Diabetes Program), Healthy Heart, American Cancer Society Fresh Start (Smoking Cessation Program), and "Get the Facts in 3 Easy Bites" (Nutrition Classes). All patient educations classes are offered for free to patients and their support systems. As an incentive, patients are given free medical supplies that are needed for self-monitoring (ex. Glucometer, automatic blood pressure machine) in class.

Programs at Christ Clinic include:

? Primary Care ? The primary care program provides access to healthcare

for acute illnesses, chronic disease management and preventative care services. We provide medical visits with a doctor or mid-level provider at minimal cost to the patient and the community.

? Acute Aftercare Clinic ? The aftercare clinic is open Monday through

Thursday from 3:00pm to 7:00pm to give an alternative to uninsured patients who frequently utilize the Emergency Room as their primary care provider (PCP). Patients are treated for the acute condition and individually educated by a Social Worker on the importance of a primary care medical home as well as being referred to a primary care medical home such as AccessHealth in Brookshire, West Houston Clinic or Christ Clinic.

? On-Site Labs - The on-site lab reduces patient barriers (such as transportation,

high cost and additional time off work) from interfering with precise, accurate and comprehensive care. Patients are asked to cover the cost of their labs.

? Pharmacy and Medication Assistance - Medication assistance is a critical

piece of quality care for those who are uninsured. A patient who receives a diagnosis and treatment plan needs to have equivalent access to necessary medication. The pharmacy and medication assistance program provides access to medications at low or no cost to patients through a multifaceted approach.

Message From The Executive Director

Christ Clinic remains a critical part of the healthcare system in our area, providing help to those living without access to affordable health care, preventative services, and prescription assistance. Over the last few years, the nation's healthcare landscape has begun to

change dramatically. Many people may be wondering if there will still be a need for Christ Clinic following the implementation of the Affordable Care Act. The answer is Yes! Millions of people living in our state will remain uninsured. This includes a significant number of our areas residents who are our neighbors, friends, relatives, and those whose work we depend on to support our local economy.

Daily, we hear heartbreaking stories from patients about how health insurance remains unobtainable even though those stories are not heard in the mainstream media. Thanks to our generous donors we are meeting the needs of our patients and have been able to grow dramatically as our area's need grows as well. We are honored to serve and to be supported by such an amazing community.

Why We Do What We Do

"Awalking time bomb"is how Claudia Mayo describes herself before she found out about Christ Clinic. Her own health issues had taken a back seat to caring for a grandson and a mother with dementia.

When she went to the emergency room with crushing chest pains and shortness of breath, doctors diagnosed her with coronary artery disease, a complication of her diabetes and high blood pressure. Because Ms. Mayo is uninsured, she was unable to receive the cardiac catheterization and coronary artery stent that she needed to treat her condition.

Ms. Mayo came to Christ Clinic looking for a miracle that would make her well again. Her family was able to find a cardiologist in our Katy community that worked to lower the cost of her needed care. She was also able to qualify for a program at Houston Methodist West Hospital that covers the cost of emergency care for low income patients, and was able to receive the coronary artery stent that she needed. But the real miracle happened when the providers at Christ Clinic shared with her that the stent was only the first step toward better health. As Ms. Mayo learned that sustained health required changes in her diet, exercise and medication compliance, she embraced a radically new lifestyle that produced miraculous results. When she was first encouraged to exercise she could not walk to the end of her block. She now routinely walks 3 miles at a brisk pace and has completed a family 5K race at her church. She eats a healthy diet, has improved energy, and no longer experiences chest pain even with exertion.

Ms. Mayo's story has inspired other patients to start their journey toward a healthier lifestyle. And so the miracle continues!

Christ Clinic ended its 2014 fiscal year in very good financial condition with positive cash flow thanks to the support of our community. The 2015 fiscal year has significant increases in patients accessing care at Christ Clinic that require increased funding. This allows the clinic to continue delivering its high quality healthcare and health related services to the most vulnerable people living and working in our area. Christ Clinic represents an excellent investment in our community with 86% of funding going directly into programs and services benefitting our patients.
