A.Obtain and Assess Data to Develop Measurable Goals and Track Progress Towards Ending Homelessness in FloridaA.1.To identify gaps and measure progress towards these goals, the Office on Homelessness will obtain information from each Continuum of Care and other local partners regarding resources administered at the community-level, including:XXXXA.1.a.Amount of federal Continuum of Care Homeless Assistance GrantsA.1.b.Allocation of Homeless Assistance Grants to permanent supportive housing, rapid rehousing, transitional housing, coordinated entry, planning, Homeless Management Information Systems and other purposes; this information will be updated at least annually A.1.c.Amount of Support Services for Veteran Families grantsXA.1.d.Amount of Veterans Administration Supportive Housing (VASH) vouchers awarded to each public housing authorityXA.1.e.Amount of Emergency Solutions Grants awarded to participating jurisdictionsA.1.f.Amount of Housing Opportunities for Persons with AIDS awarded to participating jurisdictionsA.2.Included in this data will be the targeting of each programA.2.a.What populations are each of the permanent and transitional housing programs serving (chronic, non-chronic, individuals, families with children, youth, veterans, other)XXXXA.2.b.How many permanent supportive housing units are serving formerly chronically homeless households; how long have the residents resided in the unit; how many residents would likely be able to exit permanent supportive housing if another subsidized housing unit were securedA.3.The Office on Homelessness will collect data to report how communities are progressing in efforts to end homelessness among veteransXA.4.The Office on Homelessness will have each Continuum of Care utilize the US Interagency Council on Homelessness Supportive Housing Opportunities (SHOP) Tool to determine what specific combination of strategies are needed to increase the availability of permanent supportive housing in their community and submit that information to the Office for use by the Council in developing its plan. A.5.In recent years Florida Housing Finance Corporation has dedicated resources to financing housing to serve special needs and formerly homeless households. Florida Housing will quantify how many of these units are restricted to serving formerly homeless and chronically homeless personsA.6.The Department of Economic Opportunity, with CareerSource Florida, will gather baseline data on workforce development programs, including: A.6.a.Use DEO’s client management systems to identify the number of homeless persons served by the state’s workforce programsA.6.b.Survey Local Workforce Development Boards (LWDBs) to identify local efforts to serve homeless clients through outreach, networking with other local agencies, special projects, etc.A.6.c.Identify current practices being implemented to serve homeless veteransXA.6.d.Review workforce policies, procedures and regulations to identify existing barriers that hinder access to program services by jobseekers experiencing homelessnessA.7.The Department of Children and Families will monitor how many Link to Permanent Housing Units are utilized by agencies funded through Community Based Care and Managing Entity organizationsB. Increase Leadership, Collaboration, and Civic Engagement B.1.The Council will work with member agencies to coordinate state and local resources and technical assistance to increase nonprofit capacity to develop and operate a range of affordable rental housingB.2.The Council and its member agencies will continue to support the permanent supportive housing pilot underway; using the pilot as a mechanism to identify barriers to efficiently and effectively linking supports with housing for high utilizers of crisis servicesB.3.When funding or other opportunities arise that require coordination or input across multiple sectors, the Council will convene stakeholders to efficiently and effectively develop approaches to optimize the opportunity and increase capacity throughout the state to end homelessnessB.4.The Council will adopt, and member agencies will utilize, a formal definition of supportive housing as developed by Florida State Supportive Housing WorkgroupB.5.State agencies will formalize through Memorandum of Understanding the Florida State Supportive Housing Workgroup to coordinate the increased production of permanent supportive housing and report outcomes to the CouncilB.6.Each state agency will access whatever federal resources targeted to homelessness or persons experiencing or at risk of homelessness are made available; this includes grants, technical assistance, participation in initiatives and learning about and implementing evidence-based and best practices; updates as to status will be provided at each quarterly Council meeting.C.Increase Access to Stable and Affordable Housing C.1.The Council and its member agencies will adopt the US Interagency Council on Homelessness Housing First philosophy in implementing actions to achieve the Council’s goalC.2.By December 2016, the Council will establish a permanent supportive housing production goal to achieve an end to chronic homelessness by 2021. This goal will take into account units funded, or reasonably projected to be funded, through the Continuum of Care Homeless Assistance Grants as well as units made available through the turnover of existing units; this goal will include recommendations for targeting resources to create new units and ensure residents have appropriate supportive housing servicesC.3.The Council will use this data to develop a recommendation as to the resources and best practices necessary to meet this production goalC.4.The Department of Children and Families will assess best practices and amend its Temporary Assistance for Needy Families (TANF) program to increase implementation of a Rapid Rehousing strategyC.5.The Department of Children and Families will amend its Emergency Solutions Grant (ESG) to increase implementation of a Rapid Rehousing strategy, including a revision of its contracting process to allow for multiyear grantsC.6.The Office on Homelessness will identify gaps across the state to implementing Rapid Rehousing strategies, including: identifying capacity development and technical assistance needs to implementing Rapid Rehousing and integrating it with each community’s coordinated entry system; potential incentives for transitional housing providers to convert to permanent housing; determining funding needs to increase Rapid Rehousing capacity throughout the stateC.7.The Department of Health will implement initiatives which reduce the risk of homelessness amongst people living with HIV/AIDS and their families and high-risk tuberculosis patientsD.Increase Economic Security D.1.The Department of Economic Opportunity and CareerSource Florida will D.1.a.identify and disseminate best practices from workforce research and literature, LWDBs and other local partnersD.1.b.provide training and technical assistance to LWDBs and local partners on best practices and employer incentives to hire individuals with barriers, such as jobseekers experiencing homelessnessD.1.c.monitor funding opportunities and apply directly, or assist local partners to apply, for grants to serve homeless clientsD.1.d.consult with LWDBs and workforce partners to identify future actions and/or recommendations to increase economic security for homeless FloridiansD.2.The Department of Children and Families will work with CareerSource Florida to identify ways Rapid Rehousing programs can be integrated with employment and workforce development services; and assist and advise in the creation of partnerships between philanthropy, community services and local and state funded programsD.3.The Department of Education will ___ (best practices, data, early learning, elementary, secondary, post-secondary, continuing education, grants, etc)D.4.The Department of Health will D.4.a.Implement school health services program to ensure that students are healthy, in school, and ready to learn. D.4.b.Support dental sealant school program initiatives to effectively prevent tooth decay in children and reduce school absenteeism related to oral health issues. D.4.c.Collaborate with Department of Agriculture and Consumer Services, Department of Children and Families, Department of Education, and Early Learning Coalitions to disseminate resources and information that promote healthy weight in early child care facilities. D.4.d.Implement the Afterschool Meals Program and the Homeless Children Nutrition Program. D.4.e.Sustain the Veterans Application for Licensure Online Response (VALOR) system to help veterans obtain gainful employment XD.5.The Department of Veterans Affairs will work with the U.S. Department of Veterans Affairs and local Continuums of Care to identify gaps and barriers to ending homelessness among veterans in FloridaXE.Improve Health and Stability E.1.The Agency for Health Care Administration will E.1.apromote partnerships between Medicaid plans and housing resources E.1.b.monitor adherence with appropriate discharge policies from health care facilitiesE.1.c.analyze current Medicaid resources and identify cost effective solutions for providing services in permanent supportive housing E.2.The Department of Children and Families will E.2.a.promote Managing Entities knowing their clients that are experiencing homelessness E.2.b.promote connecting persons experiencing homelessness that are discharged from healthcare facilities with appropriate resources E.2.c.analyze current resources and identify cost effective solutions for providing services in permanent supportive housing E.2.d.apply for federal technical assistance and grants that can be used to reduce and end homelessness E.3.The Department of Corrections willE.3.a.promote connecting persons experiencing homelessness that are discharged from healthcare facilities with appropriate resourcesE.4.The Department of Health will E.4.a.Improve access to HIV treatment and other healthcare support for people living with HIV/AIDS through the administration of the Housing Opportunity for Persons with HIV/AIDSE.4.b.Collaborate with hospitals to implement the Florida System of TB Care E.4.c.Provide re-entry programs assistance with access to vital records, such as birth certificates F.Retool the Homeless Crisis Response System F.1.The Office, with input from the Council, will assess the feasibility and efficacy of creating a “balance of state” Continuum of Care to serve rural and underserved communities. F.2.The Office on Homelessness with utilize the Performance Measures as outlined in the US Department of Housing and Urban Development release of guidance on McKinney-Vento Homeless Assistance Act to assess the performance of each community in implementing strategies to reduce and end homelessness F.3.The Office on Homelessness will retool its reporting structures to better monitor progress towards these goals; and track and annually update the Council on how each community does with regard to three key outcome measures: F.3.a.Number of persons that are homeless F.3.b.Number of persons that exited homelessness F.3.c.Rate of recidivism back to homelessness F.4.The Office on Homelessness will work with local partners and the Department of Financial Services to develop more efficient methods for administering and monitoring contracts.F.5.State agencies will utilize outcomes data related to reducing homelessness and implementation of best practices related to assisting persons experiencing or at risk of homelessness in all funding decision; this includes contracts with Continuums of Care, Managed Care Organizations, Managing Entities, Community Based Care Organizations,_____... ................
