IMPLEMENTATION TEAM APPLICATION PROCESS INSTRUCTIONS

PROJECT OVERVIEW

The goal of CODIE is to build the capacities of schools’ district- based CODIE implementation teams to achieve proficiency in facilitating Discovery, integrate Discovery into their transition processes, and train other staff so any student can access the Discovery process for transition support.

The CODIE model incorporates the values of the Discovery process and customized employment that focus on individualization, community inclusion, educational enrichment, self-determination, interagency team collaboration, and social capital development that lead to gainful competitive, integrated employment. This upholds the pillars that make education services an effective, outcome-oriented process. This approach to transitioning youth offers the promise of welcoming all persons, with life complexities who wish to work, into employment.

Through this application process, two school district led implementation teams will be selected. If your district is selected, you will receive $1,000 to support direct costs associated with CODIE and capacity building, training and technical assistance that will focus on:

▪ team building and cohesion activities,

▪ integrating Discovery into exiting transition processes such as: Florida’s Access Points Standards and IDEA’s Indicator 13,

▪ developing team processes and procedures for educating students and their families about Discovery,

▪ ensuring fidelity of the Discovery model,

▪ district team members attaining proficiency as Discovery facilitators,

▪ recruiting 2 to 4 students with developmental disabilities to complete Discovery in Year , and

▪ participating in a web-based community of learners group site.

Applications are due November 7, 2012

SUBMISSION PROCESS

ELIGIBLITY

Florida school district personnel must be the lead applicants in completing and submitting a CODIE application. All Florida school districts are eligible except Pasco and Lee who are Year 1 CODIE Implementation Teams.

COMPLETING THE APPLICATION

▪ Section A: Implementation Team Contact Person

This section must be completed in order for your application to be considered.

▪ Section B: Implementation Team List (15 points)

Selection of your team members is integral to your success. In this section you must provide the name, position/title/affiliation, telephone, and e-mail for each implementation team member.

o Required team members: A district level ESE supervisor (e.g. district transition specialist, ESE coordinator, or secondary education specialist, etc.) and educators from at least two high schools.

o Recommended team members: school transition specialists, school job coaches, school administrators, Project Search personnel, vocational rehabilitation personnel, employment specialists, adult service providers, Project 10 staff, SCIETT members, and other related community partners.

▪ Section C: Questions (80 points)

Please provide detailed, thorough responses to these questions. It is important that you provide comprehensive answers. You have the option of answering the questions on the existing application page and using as much space as you need OR creating a separate document to attach to the application.

▪ Section D: Training needs (5 points)

Complete the entire training needs chart while taking into considerations the needs of all of your team members.

▪ Section E: Commitment to Participate as CODIE Implementation Site

This section must be signed by an ESE school district level supervisor and a transition supervisor who are listed in Section B in order for your application to be considered.

SCORING

Completed applications will be scored on scale of 100 points. Sections B, C, and D are worth a total of 100 points; however, if Section A is not completed and Section E is not signed by the school district supervisor and transition supervisor, who are listed in Section B, your application will not be considered. Two applications with the top scores will be selected as Year 2 CODIE implementation teams are notified by October 29, 2012.
