How It Works
Scheduling
• Consider scheduling at the following times:
o During business hours
o During staff meetings or committee meetings
o At evening gatherings
o Before school, during lunch time, or after school
o Between church services
o At special events
• After scheduling, the nurse will contact you approximately 3 days to 1 week prior to your clinic. Please confirm the expected number at that time.
Announcement Fliers, Sign-up Sheets, Consent Forms and VIS
• Announcement fliers, sign-up sheets, consent forms, and the Vaccine Information Statement (VIS) will be sent to you prior to your clinic. These forms can also be downloaded from our Website under Materials For Your Clinic.
o Announcement fliers can be modified to meet your needs.
o You can maximize the number of participants by having them sign up ahead.
o Each immunization recipient must complete a consent form prior to receiving an immunization. Making consent forms available for completion prior to the nurse’s arrival, streamlines the clinic. The nurse will also have copies of these forms on hand.
o CDC Vaccine Information Statement (VIS) include current information about the vaccines. They are to accompany consent forms.
o For proof of vaccination, upon request, receipts can be made available to individuals at the time of the clinic.
Keep In Mind
• Shots are given in the upper arm, so recipients might want to give thought to the shirt
they wear.
• First-time flu shot recipients are advised to remain in the company of the nurse for 15 minutes after receiving the shot; therefore, it is recommended that they come toward the beginning of the schedule.
Set Up
On the day of the clinic, please provide each nurse and clerk with the following:
• A table
• Two chairs
• A trash can
Payment

• Cash, check or credit card at the time of service.
• Invoice option for companies.
• We submit claims for accepted insurances.
Age Policies
Flu Shot
• Must be 4 years of age or older to receive a shot.
High-Dose Flu Shot
• Must be 65 years and older to receive a high dose flu shot.
