Vendor Fingerprint Authorization Letter

The possessor of this Vendor Fingerprint Authorization Letter is an employee of an information technology vendor contracted to complete work on Pennsylvania State Police (PSP) network systems that have access to the Commonwealth Law Enforcement Assistance Network/National Crime Information Center (CLEAN/NCIC) and/or Criminal History Record Information (CHRI). The vendor/contractor employee must take this Vendor Fingerprint Authorization Letter to a PSP Station in order to be fingerprinted. The vendor/contractor employee must provide a valid photo driver's license, passport, or other acceptable photo identification, along with a vendor identification card or other proof of employment when they appear for fingerprinting.

Fingerprinting shall be completed at any PSP Station. A complete list of PSP Station locations is available on the PSP public website at under the “Public Services” tab. The vendor/contractor and/or their employee must contact the appropriate PSP Station to schedule an appointment to be fingerprinted. The fingerprinting process will take approximately 30 minutes.

Procedures for Fingerprinting Vendors

Live Scan/electronic fingerprinting machine procedures:

• Select “Applicant” from menu. Applicant type: Criminal Justice Employment.

• Originating Agency ORI: Use live scan default.

• Controlling Agency Identifier: PAPSP0015.

• Enter demographics. Roll fingerprints.

• State and Federal responses will be returned to both ORIs listed.

Ink procedures (only used if a live scan machine is unavailable):

• Roll fingerprints on a blue and white FBI Applicant Fingerprint Card, Form FD-258.

• Ensure all mandatory fields listed below are completed.

❖ Mandatory Fields:

▪ ORI: PAPSP0015—PSP CLEAN/ADMIN SECTION.

▪ First and Last Name, DOB, SSN. All identifiers (e.g., sex, race, height, etc.).

▪ Home Address, Present Employer, and Employer's Address.

▪ REASON FINGERPRINTED: “Criminal Justice Employment.” NOTE: This must be hand printed on the FBI Applicant Fingerprint Card.

In those instances where a live scan machine is unavailable, the completed fingerprint card and Vendor Fingerprint Authorization Letter shall be mailed to: PSP Fingerprint Card Query Mailing Unit, 1800 Elmerton Ave., Harrisburg, PA 17110, by the PSP personnel taking the fingerprints. The PSP Bureau of Records and Identification will process the completed fingerprint card, and the responses will be sent to the submitting Station and the CLEAN Administrative Section, Bureau of Communications & Information Services.
