Some essential points:

There are two basic formats of SDR problems; one asks you to find the safe dose range per dose, and the other asks you to find the SDR per day. When you read the problem for the first time, determine what the problem is asking you to do.

Next, if the weight is given in pounds, convert it to Kilograms, since all SDR use Kg. as opposed to pounds.

Be aware that some SDR are stated as the SDR per dose or per day, without the need to multiple by Kg, at all. For example, the maximum dose for Acetominophen for an adult sized child (>50kg) per day is 4Gms. There is no need here to multiple by Kg.; it is a statement of maximum dose per day.

However, the SDR of Acetominophen per dose is found by multiplying by Kg. For example, the SDR for a child less than 50KG is 10-15mg/Kg/dose. Notice, this is per dose. Therefore, if a child weighs 12Kg and you are asked to find the SDR for Acetominophen, you would multiple:

10(12) and 15 (12) to get 120mg to 180 mg per dose.

Notice, we no longer say per Kg, in this range of SDR because we have already multiplied by Kg, therefore dropping the label of Kg. Essentially, we have converted mg/kg/dose to just mg/dose (mg per dose).

Now, try these problems for yourself:

1) Find the SDR of Ampicillin per dose, when the SDR and wt are given to you:

SDR: 100-200mg/kg/24hrs divided Q 6 hrs.

Wt: 35 lbs.

Convert lbs to Kg:

Multiply the SDR by the Kg:

This is per day, so now divide the SDR per day by the number of times the child receives the medication. A dose given Q 6hrs is 4x/day. So, divide the minimum and the maximum SDR by 4.

You will now have your SDR by dose for a child of 35 lbs.

2) Find the SDR of Gentamicin for a child who weighs 22lbs.

The SDR for Gentamicin for child less than 50Kg = 6-7.5mg/Kg/24hrs Q 8hrs.

Convert lbs to Kg:

Multiple the Kg by the min. and max SDR, resulting in mg/24hrs.

Convert the SDR from per day to per dose:

This should be your SDR per dose.

3) As a nurse, you don’t have to write the order for the appropriate dose of medication, but you do need to know how to check it against what it should be. The above two examples show you how to calculate what it should be. Now, consider a child who weighs 35 lbs with an order for Ampicillin 500mg IV Q 6hrs. Your job is to determine if this is within SDR. Show your work:

4) Likewise, you have a child who weighs 22lbs who has an order to receive Gentamicin 80 mg IV Q 8 hrs. Determine if this dose is within SDR for this child. Show all work:

5) Medication knowledge: What are the side effects that are a problem with Aminoglycosides?

6) Medication knowledge and critical thinking: Before giving Gentamicin, or any Aminoglycoside medication, you would want to check what type of lab data?
