|SUBJECT: |CARE Change Control Information – June 27, 2014 |

|Purpose: |To provide information about the CARE Change Control Release on Friday, June 27, 2014. |

|Background: |This CARE Change Control Release will occur on Friday, June 27, 2014 starting at 5:35 pm. CARE users will receive the |

| |new version when they log into CARE any time after 8:00pm on Friday. |

| | |

| |No action is required by users. CM/SSS/CRMs should check their cases in and close CARE before leaving for the weekend |

| |on June 27, 2014. |

|What’s new, changed, or |The release will include: |

|Clarified |DDA & HCS/AAA : |

| | |

| |DB Settlement/Limitation Extension |

| |A statement with information about how clients or their representatives can request a Limitation Extension has been |

| |added to the CARE Service Summary (HCS/AAA) or ISP (DDA) for clients whose assessed age is under the age of 21 and who |

| |receive personal care services. |

| |Relevant Limitation Extension (LE) ETR Categories and Types have been added to the ETR screen functionality in CARE. At |

| |this time, LE requests will be entered into the CARE ETR screens by HQ staff after receiving request and documentation |

| |materials from the client or client/rep and/or CM/SSS/CRM. |

| | |

| |*Note: DDA will be releasing a separate joint MB this month with more detail regarding the DB Settlement/Limitation |

| |Extension policy and procedure. |

| | |

| |M.R. v Dreyfus Settlement Agreement: In-Home Personal Care Base Hours for Named Individual Plaintiffs Identified in the |

| |M.R. Lawsuit Agreement |

| |Personal care hours will calculate automatically in CARE using base hours that are at least as great as the base hours |

| |in effect in December 2010 for each of the named plaintiffs in the lawsuit agreement. |

| |Named plaintiffs will have their CARE record flagged and identified on the Client Demographics screen so that the |

| |correct base hours can be applied for in-home personal care hours. |

| |A CARE Informational dialogue box will appear if “In-Home” is selected in the “Living Situation-Planned” drop down on |

| |the CARE Plan screen for an individual who is a named plaintiff in the lawsuit settlement agreement. |

| |A statement will be added to the services PAN for named plaintiffs receiving in-home personal care. |

| |There are changes in the CARE application and algorithm to determine if an individual is eligible to become a client of |

| |DDA. |
