This letter is for everyone that makes an appointment with CCCS for our counseling services. Listed below is the information required for the specified counseling sessions. Our Budget Information Worksheet must be filled out before your appointment or we may reschedule your appointment due to time constraints. If you need a counseling session by phone please mail the information required for the specified counseling session with a copy of your driver's license and the fee to our office; once we receive all of the information required we will call you to schedule a counseling session by phone.

WHAT TO BRING FOR YOUR CREDIT COUNSELING SESSION OR CREDIT REPORT REVIEW:

Completed Budget Information Worksheet (4 pages), signed Statement of Counseling Services and Privacy Policy.

Make sure to list all debts (contact information, addresses and phone numbers are helpful). If you are not sure of your debt situation you may wish to obtain a copy of your credit report. You can get a free credit report, once a year, at or call 877-322-8228.

Current Statements from your creditors and/or collection agencies. Valid Government issued Photo ID (Driver's License, State ID, or Military ID) For a Credit Report Review please have your previous address, credit card account numbers, names of creditors,

and loan payments and all of the above paperwork completed.

WHAT TO BRING FOR PRE-FILING BANKRUPTCY COUNSELING:

Completed Budget Information Worksheet (4 pages), signed Statement of Counseling Services and Privacy Policy. Make sure to list all debts (contact information, account number, address and phone numbers are helpful).

Valid Government issued Photo ID (Driver's License, State ID, or Military ID)

WHAT TO BRING FOR A DELINQUENT MORTGAGE/RENT:

Completed Budget Information Worksheet (4 pages), signed Statement of Counseling Services and Privacy Policy.

No Fee is required for Delinquent Mortgage Counseling. How many months are you behind? What type of home loan do you have, FHA, Conventional, VA, etc.? Do

you want to keep your home? What payment arrangement can you make? Why are you behind on your mortgage? All mortgage lender statements and/or attorney paperwork, and current Insurance company information. Current Statements from your creditors and/or collection agencies. Valid Government issued Photo ID (Driver's License, State ID, or Military ID) If you rent or lease please bring a copy of your lease/rental agreement.

WHAT TO BRING FOR A REVERSE MORTGAGE:

Completed Budget Information Worksheet (4 pages), signed Statement of Counseling Services and Privacy Policy.

No Fee is required for a Reverse Mortgage Valid Government issued Photo ID (Driver's License, State ID, or Military ID) and your Social Security Card.
