[Insert the School’s Name or Logo]Staff Laptop Computer User AgreementConditions Of Issue (Name of the School) STAFF LAPTOP COMPUTER PROGRAM offers each teacher the opportunity to use a laptop computer from the school for the time they are employed at the school. [Statement of Program Aims ] The provision of the Laptop computer is subject to the following conditions:The Laptop computer is provided on a ‘use’ basis and remains the property of the legal proprietors of (Name of the School). The school wil provide the insurance coverage for the laptop. The usage cost will be $XXX per fortnight. This amount will be deducted from your pre-tax salary.The provision of the Laptop computer is related to current employment and could change if conditions of employment change. The Laptop computer is always the property of the school and must be supplied to the Principal or authorised representative when requested, either:a) in “as is” condition immediately upon request;b) or, if nominated by the school with all personal files and programs removed: i. within 24 hours of an official request in writing by the School, or not less than 24 hours prior to departure from the School subsequent to cessation of employment, ornot less than 24 hours prior to the commencement of leave taken for the purpose of maternity, extended sickness, leave without pay or similar situations. (Any variation to these arrangements will be solely at the discretion of the Principal. Failure to return the Laptop as in 3bii) and iii), will delay payment of final entitlements).The Laptop computer will be used in accordance with all Codes of Conduct, as established by the School, or governing authority, presently, or in the future, in particular the Acceptable Use Policy and Staff Policy for Use of Internet and Email, which includes strict compliance with the Australian Copyright Act 1968.Responsibility for maintaining backup copies of personal files resides with the staff member. In the case of treatment of serious hardware or software malfunction, the (IT Coordinator), while taking every care, cannot guarantee restoration of the files and programs back to anything other than that of a standard staff Laptop computer image. Installation of personal software, other than what is contained on the standard image (programs), is the responsibility of the Staff Member.All damage and faults will be reported to the (IT Coordinator) within a reasonable time to permit repair under Warranty conditions.No unlicensed software may be installed onto the Laptop computer, and any single user license, that is the property of the school may not be loaded onto more than one laptop computer at any time. All software must be loaded by the IT Coordinator.The School accepts no responsibility, and cannot guarantee support, for any unofficial (i.e. not part of the standard image or specifically required for documented curriculum purposes) software, data, etc. installed onto the Laptop computer.Major reconfiguration of the Operating System, (eg. alterations to autoexec.bat), or to the Configuration System, may be treated as negligence if undertaken without due care.a)Security of the Laptop computer is at all times the responsibility of the Staff Member. In particular it should:not be left in unattended offices or classrooms,not be left in a situation so as to permit student access,be stored in a secure, safe location when used outside school or during transportation.not be left in a visible place in a motor vehicleb)The physical safety and care of the Laptop computer also needs to be managed appropriately:the machine must always be transported in the purpose built case,temperatures should be monitored as laptops are heat sensitive,they are not to be used in the vicinity of food, drinks or chemicals,they should not be cleaned with any products other than those supplied by the (IT Coordinator).c)Password protection and network security must also be maintained. Staff will adhere to the (IT Coordinator) strategies introduced to protect the integrity of the network, including password protected screensavers, password changes, remote access guidelines etc.The Staff Member agrees to pay the cost of of insurance excess for loss or damage that may be caused to the Laptop computer through negligent or careless use that is outside warranty conditions and not attributable to normal wear and tear. In such instances, a Statutory Declaration stating the circumstances surrounding the loss or damage, is required to be completed. Any decision concerning obligations under this Clause will be made by the Principal and will be based upon this Declaration.All reference to the Laptop computer includes Radio-LAN card, carry case, manuals, mouse and other School owned peripherals (if supplied).[Name of the School]Staff Laptop Computer ProgramDeed Of Agreement & Request to Salary Package a Laptop ComputerThis Deed of Agreement is made between the legal proprietors of (Name of the School), AND……………………...………………..
