In person: Bring completed form, supporting documentation and desired payment method to Melbourne Town Hall 120 Swanston Street Melbourne VIC 3000 Monday to Friday 7.30am to 5pm.

As part of this application you’ll receive a lifetime registration tag for your pet. Replacement tags are not sent annually. However, if your tag is lost or damaged, contact us at animal.management@melbourne..au and we can send you a free replacement.
