Letter to Household about Termination of Food Assistance/TAF/FDPIR/Foster Child Eligibility

Dear :
We are required to make sure that only eligible children receive reduced price or free school meals. Selected households have to show information to prove eligibility for reduced price or free meals. Your household’s application has been selected to be verified.
Your child’s eligibility for free school meals was based upon the case number reported for Food Assistance, Temporary Assistance for Families (TAF), Food Distribution Program on Indian Reservations (FDPIR) or foster child status. However, official records indicate that your household was not eligible for these benefits at anytime from when you applied for free school meals to the present.
You must now prove eligibility based on household size and income. The enclosed page, Confirm Your Eligibility for Free/Reduced Price Meals, shows the kinds of information or documents that you may use to show your household’s eligibility. If you give us original papers, we will make a copy and send the originals back to you.
Bring or send proof of eligibility to the school by .
