A MESSAGE FROM THE ACULABS MANAGEMENT TEAM: To avoid unwarranted denials and subsequent rework for office staff, order forms must include an ICD-10 Code or patient symptoms. These symptoms or ICD-10 codes are required by certain thirdparty payors to confirm the medical necessity of the test(s) and/or profile(s) ordered. The ordering physician or other authorized party is responsible for providing codes that support the medical necessity of every and every test ordered in the diagnosis and treamtent of the patient. In order to assist client facilities in avoiding denials, we've put together a packet that is included with every lab book given to each individual nursing station unit. Included in this packet are specific ICD-10 codes most commonly used by Aculabs clients. These codes were taken from the 2015 ICD-10 Manual issued by the American Medical Association. While this list may be a useful tool, it is ultimately not meant to be a definite reference guide. The ICD-10 CM should be referenced for a complete list. If you need more information, please refer to the AMA's ICD-10 Manual for a complete listing. The ultimate responsibility for correct coding lies with the ordering physician.
