The following information is general information about some of the forms in your orientation packet and general procedures you should know.

• The parent conference form and incident form can be used with the parent and child to discuss and record a child’s behavior or an incident that happened.

• The progress report can be filled out by staff to help give the parents an idea of how their child is progressing in the program.

• The behavior incident form for the youth can be used with the older children as an alternative discipline technique, rather than giving a “time-out”, or as part of a time-out period.

• The health incident report is used to record injuries and/or illness. One copy is given to the parent, another copy must be signed by the parent and kept on file with FF&G.

• The FF&G survey is given to parents and/or older children to fill out once or twice a year.

• Volunteers should not be counted in the staff/child ratio unless they have had a full background check and are pre-qualified as an aide, or a higher position. Do not leave volunteers alone with students. Volunteers that are under age 15 are counted as an additional child, in the ratio.

• The mileage reimbursement form is to be filled out and turned in to your director to reimburse you for use of your personal vehicle for FF&G business. Children may be transported in your personal vehicle. In that case, they are covered under your vehicle’s insurance, as is any guest in your car. They must still be put in a seatbelt or booster seat, appropriate for their age and size.

• All teacher/group leaders are responsible for planning activities for their group based on your site’s schedule. You must turn in your plans along with your supply list at the beginning of each month. The calendar provided can be used to record your activities.
