ARD/IEP Meeting Script (Annual/REEDs)

Introductions: “Hello, my name is (your name). I am (student’s name) case manager. I would like each of you to introduce yourself and your role in this meeting.”

IEP Professional Statement: (Name of Administrator), will you please read the Professional Statement?

Purpose of the Meeting: The purpose of today’s meeting is to review the IEP for (student’s name). This meeting will serve as (student’s name) (Annual and/or Re-Evaluation) IEP Meeting.

Establish Time Parameters: In order to have an effective, productive meeting today we have scheduled (amount of time? Ordinary=: 1 hour, Difficult situations= MAX 2 hours) to complete this meeting. The time is now (state the current time). (Name of Administrator) will be our timekeeper for this meeting. If the meeting has not been completed in this time frame, we will have reconvene as soon as possible.

Student-Led Presentation: (Student’s name) will present their Student-Led IEP Power-point that he/she prepared with (Case Manager’s name) prior to this meeting. Following (student’s name) presentation he/she will be given the option to stay for the rest of the meeting.

After presentation, allow a few minutes for IEP Team members to ask questions about (student’s name) presentation.

Ask (student’s name) if they would like to go back to class or stay for the rest of their IEP Meeting.

Review of Evaluation Data: If this is a REED, have LSSP (and If applicable: SLP/OT/PT/APE, etc.) present assessment information and make recommendations for further assessment.

IEP Progress Report: Review current progress toward IEP goals. If student receives Speech and/or Related services, ask SLP and/or Related Service providers to present their progress report. Provide parent with a copy. Ask of there are any questions or comments.

Present Levels of Academic Achievement and Functional Performance: What are (student’s name) present levels of performance? Include strengths and weaknesses, grades, benchmark data, previous state assessment data, SRI scores, lexile scores, progress monitoring data, etc.

Transition: Transition is the process of moving from one stage to another. Transition services for students with disabilities begin at Elementary School and continue through High School, always moving toward life after graduation. State law requires that this process begins prior to the student’s 14th birthday. Give the parent a copy of the NEISD Transition brochure.

Review Behavior (if applicable- FBA from Review 360 and develop FBP): Address applicable sections.

Communication/Physical/AT Needs: Address applicable sections.

Establish and Revise IEP Goals and Objectives: Present recommendations for IEP Goals and Objectives including Speech and/or Related Service (s) if necessary. Ask for committee input.

Once agreed upon change from draft to “accepted by” committee for each goal.

Accommodations/Modifications: Review the proposed Accommodations/Modifications.

STAAR: (Student’s Name) is recommended to take the (version) STARR test with the following accommodations: (state the accommodations). If student is recommended to take Modified or ALT, address Participation Requirements sheet and discuss the implications for graduation if the student takes a modified or ALT assessment.

Schedule of Services and Classroom Supports: Address applicable sections.

ESY (IF applicable): If the student qualifies for ESY, address the ESY supplement.

Questions/Comments: Are there any questions or comments about the items discussed or are there any items that were not addressed in this meeting?

Assurances: Ask (name of administrator) to read the assurances.

Signatures: Obtain signatures from all Team Members present at the end of the meeting. Make sure that the parent signs the meeting first.

Prior Written Notice: We have discussed a lot of items in today’s meeting. By law you are provided 5 days to think about the decisions made in this meeting before services can be implemented, or you have the right to waive the 5 days and services may begin immediately following the conclusion of this IEP Meeting. Would you like to waive the 5 days? If yes, have the parent sign the PWN page. If no, then explain to the parent that the meeting will be locked today and they will have 5 days to review the paperwork and within the 5 days, if they have any questions about the items, they will need to contact you to address the questions. Otherwise, services will begin in five school days following the IEP Meeting.

Conclusion of the meeting: Ask the parent if they would like to stay and wait for the copy of the archived meeting, or would they like you to send them a copy of the meeting via email.
