POSITION DESCRIPTION

JOB TITLE: Wellness Coordinator

DIVISION: Support Services

SUPERVISOR: Medical Services Coordinator (Supervision)

FLSA STATUS: Non- Exempt

JOB SUMMARY: Provide coordination of medical aspects of assigned case load; fulfills wellness, both physical and mental health, role of Support team, conduct at minimum twice monthly onsite monitoring

EDUCATION OR TRAINING REQUIREMENT: Associates Degree in Social Service field; or certified CNA or MOA; Experience may be substituted for education with Executive Director’s approval.

EXPERIENCE REQUIREMENT: Minimum of 2 years experience with MR/DD consumers or medical field, one of which must be in a direct support role

SPECIAL LICENSES, REGISTRATION, CERTIFICATION, OR SKILLS: GA Driver’s License and vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)

SUPERVISORY RESPONSIBILITY: None

ESSENTIAL JOB DUTIES ANDRESPONSIBILITIES:

- Liaison between nursing staff and program staff regarding medical supports and need of specified caseload. Document level of care and updates via email and activity notes.

- Responsible as back-up monthly (for nursing staff)check in and verification of medications for assigned case load in cooperation with the Consumer Living Home Manager or designee

- Monitor medication on an ongoing basis including onsite monthly monitoring of all medication, both prescription and non-prescription.

- Coordinate with nursing staff to correct any medication receipt and medication documentation errors

- Ensure refills of PRN and prescriptions are completed correctly and in a timely manner

- Ensure medication transfer is complete for any new individuals or transfer, transition of individuals, internally and externally.

- Coordinate with Consumer Living Home Managers the scheduling of medical visits; ensure the documentation required at visits is completed; collect and monitor all medical visit forms/documentation (review and distribute), including follow up data

- Monitor allergies and dietary constraints/prescriptions for accurate and appropriate implementation as well as correct information is reflecting on all internally and externally generated documentation (including but not limited to HRST, onsite medical information book.

- Monitor and provide general oversight of bowel tracking, blood sugar monitoring, and blood pressure monitoring; provide both off sight follow up and on sight monitoring to ensure appropriate and accurate reporting and following of protocols.

- Monitor the need for or the receipt of lab work; collect data and ensure proper follow up if required.

- Responsible for completion and updating of HRST, coordinating with Community Living Advocate and Consumer Living Home Manager to obtain needed information.

- Ensure consumer rights are appropriately maintained; report violations

- As requested via supervisor attends home team meetings to provide information and training.

- Positively, and effectively interact with regional boards, consumers, families and all levels of UCP staff.

- Attend Day /Residential staff meetings, as applicable.

- Remain in active communication with supervisor on all program matters.

- Develop and maintain positive and productive relationship with consumers, families, co-workers, and community members.

- Maintain objectivity in position in order to set appropriate limits while working with consumers.

- Support consumers in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.

- Adhere to all agency policies and procedures and Personal and Private Home Care Rules and Regulations.

- Other duties as assigned.
