What problem does your pilot seek to address? Who are the potential beneficiaries and other stakeholders? What are the inputs and activities, and what are the outputs? What does success look like after 12 months of funded research? How will your solution reach the marketplace? 2500-character limitProblem StatementPrediction and early intervention can significantly reduce the economic burden of chronic and acute diseases. In India, chronic diseases account for around 60% of all deaths. High blood pressure now affects one in five adults in rural India, while diabetes affects about one in 20 adults. Chronic diseases also influence the risk of COVID-19 infection and the course of the disease. Artificial Intelligence in combination with miniaturized sensing mechanisms has the potential to provide affordable healthcare to India’s underserved by moving from merely disease detection towards disease prediction and wellness.Technical ApproachRecent scientific studies have established chronic inflammation as an indicator of general wellness and as a precursor or risk indicator for chronic diseases. Chronic inflammation and acute inflammation both have been shown to have direct bearing on COVID-19 screening and treatment decisions. Measuring and managing chronic inflammation has shown great promise for managing aging and disease. Currently available techniques for identifying inflammation and/or stress call for the use of markers that are determined via invasive tests that entail obtaining samples of blood, saliva, urine, and/or sweat from the subject. Some examples of the markers include C-reactive protein (CRP), salivary cortisol and serum proteins. However, determination of these markers suffers from numerous shortcomings. Firstly, biological samples are drawn invasively from the user in clinical settings. Secondly, since these are point measurements and are not continuous, rhythmic variation in markers are not captured. At Myelin Foundry, we are conducting trials and developing systems to non-invasively predict chronic inflammation, acute inflammation, and stress. Through the UK-India Technology Partnership Fund, it is proposed for Myelin to conduct a 200-person clinical trial to further establish the method and system for non-invasive measurement of chronic inflammation, acute inflammation, and stress. It is proposed that the Nudge Lifeskills Foundation conduct research to define a product and business model suitable for rural access and affordability. Plaksha university will be the academic partner with students assisting with the AI analysis. Tantragyaan will be the wearables partner providing prototypes for testing Myelin technologies. Dr. Sreedhar Mandayam will provide medical knowledge support on inflammation.12.What is ‘technological’ about your proposed solution and why might this be appropriate to the challenge areas?1000 character limitWe utilize deep neural networks to perform sensor data fusion of structured and unstructured user data to estimate chronic inflammation, acute inflammation, and stress. Structured data includes bio-impedance, heart-rate variability, age, and gender. Unstructured data includes pictures of the eye, nails, tongue, and voice samples. Our system has a wearable form-factor and hence facilitates continuous non-invasive measurement. Initial tests and statistical analysis show that the proposed approach is feasible. A system to continuously monitor and predict chronic inflammation non-invasively would provide users with the means to track their wellness status over time and take corrective action based on their individual risk for chronic diseases. Additionally, COVID-19 (an acute inflammatory disease) screening and disease management can directly benefit from knowing the inflammatory state of the individual. your proposed solution a response to the impacts of COVID-19, or an effort to contain the pandemic? If so, please explain. 1000 character limit Ans: Yes.COVID-19 severity has been associated with inflammatory markers. Wearables measuring physiological parameters in combination with AI have the potential to screen for early intervention or track the recovery from COVID-19. As an example, Empatica Inc., an MIT spinoff, utilizes non-invasive measurements from medical smartwatches and artificial intelligence to enable continuous and real-time insight into COVID-19 like symptoms. Our proposal is to use non-invasive measurements to estimate chronic inflammation, acute inflammation, and stress. This approach, we believe, will be more accurate as the training phase includes both blood tests and non-invasive measurements to build an accurate AI model. During the inferencing phase (field deployment) there is no further need to conduct blood tests as the model estimates the inflammatory markers.(20)30362-3/fulltext14.Does your proposed solution contribute to combatting climate change or promoting a greener planet? If so, please explain. 1000 character limitAns: Yes, though not the primary objective of our proposal.Climate change has been linked to increase in infectious disease spread. Similarly, the relation between ambient temperature and pollution with chronic disease load has been established. Non-invasive measurement of chronic inflammation, acute inflammation, and stress will enable detecting episodes of allergies, asthma, and other acute episodes as well as chronic issues exacerbated by climate change. is your proposal relevant to the development challenges of India? 1000 character limitChronic diseases are especially an issue with the urban and rural poor who lack awareness and access to appropriate and regular tests. Additionally, due to long term disability and premature deaths, chronic diseases lead to unsustainable medical expenditure for the poor. COVID-19 more severely impacts lower-income groups due to improperly managed chronic conditions and poor access to care. The technologies presented in this proposal help reduce costs by enabling early detection and managing care more efficiently. consideration have you made of gender in developing your concept? Could your project address gender inequality or other kinds of inequality? 1000 character limitWomen and minorities are often under-represented in clinical trials which then lead to gender bias in the data and models. We aim to explicitly address gender as a variable in the clinical trials by increasing representation of women and analysing the data for gender driven patterns and improving the disease prediction accuracy.: Please be kindly reminded of the primary criteria: relevance to the environmental and/or C19 agenda. See boxes 13 and 14. All boxes must be completed with the exception of Box 5In the interests of fairness, proposals that exceed the character limits will not be considered. ................
