UNITED CEREBRAL PALSY OF GEORGIA, INC.POSITION DESCRIPTIONJOB TITLE:Wellness Coordinator (LPN)DIVISION:Support ServicesSUPERVISOR:Director of Nurse Case Management (Supervision)FLSA STATUS: Non- ExemptJOB SUMMARY: The Wellness Coordinator (LPN) acts as the chief support for individual wellness in partnership with UCP Nurse Case Managers. Wellness Coordinators provide day-to-day wellness management by: arranging medical appointments,transporting and accompanying individuals to various health-related appointments,acting as the liaison between medical providers and UCP program & support teams,educating program staff on medical needs and care practices,conducting periodic health assessments (e.g., Health Risk Screening Tool, Abnormal Involuntary Movement Scale), andmonitoring medical records to ensure regulatory compliance.EDUCATION OR TRAINING REQUIREMENT: Associates Degree or diploma in Nursing from an accredited program is preferred in conjunction with current licensure in Georgia as a Licensed Practical Nurse (LPN); Extensive experience may be substituted for education with Executive Director’s approval.EXPERIENCE REQUIREMENT: Minimum 1 year experience of clinical nursing practice.Minimum 1 year experience in the field of intellectual and developmental disabilities.SKILLS REQUIREMENT:Functional ability to assess and communicate medical needs.Strong communication skills to educate lay persons on medical conditions and following plans of care.Ability to build shared understanding of – and shared commitment to – common goals.Ability to function as a strong team member by demonstrating core characteristics of openness, trust, support, and respect.Monitoring/Assessing performance of self and other individuals to make improvements or take corrective action.Giving full attention to what other people are saying, taking time to understand the points being made, and asking questions as municating effectively verbally and in writing as appropriate for the needs of the audience.Knowledge of basic computer concepts and usage of computer systems and software (Windows OS, Microsoft Office, email, etc.).Maintaining an attitude of flexibility in an environment of ongoing change.SPECIAL LICENSES, REGISTRATION, CERTIFICATION, OR SKILLS: Licensed Practical Nurse certification (current in Georgia), GA Driver’s License and vehicle insurance; must have ability to utilize Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)SUPERVISORY RESPONSIBILITY: NoneESSENTIAL JOB DUTIES ANDRESPONSIBILITIES:Serve as the liaison between nurse case managers, program staff, and medical providers on issues of medical support to ensure individual health and wellness needs are communicated and met.Document all interactions relevant to individual wellness, including – but not limited to – level of care changes, medication changes, professional treatment of medical conditions, and health issues to be included in an individual service plan.Assist nurse case managers with monthly medication check-in and verification of medications for assigned caseload.Assist nurse case managers with both routine and specially requested health and/or medication staff trainings. The LPN Wellness Coordinator may be occasionally assigned to conduct new hire health services trainings in the absence of an assigned nurse case manager.Assist nurse case managers with occasionally assigned telephonic nursing guidance for program staff.Monitor individual medication on an ongoing basis, including routine onsite medication checks.Report, and assist with correction of, discovered medication errors.Ensure prescription renewals (including annual renewals of PRN medication authorizations) are completed correctly and on time.Lead medication and medical record transfers for newly admitted individuals, or individuals who transition locations within UCP.Schedule and provide transportation for routine medical visits; ensure required documentation is completed; collect, review and distribute medical visit record forms/documentation – including relevant follow up data.Coordinate with program staff to arrange for transportation of individuals to non-routine urgent medical visits; provide post-visit follow up to ensure documentation is complete, and communicate diagnoses and treatment recommendations to nurse case manager and other appropriate team members to ensure continuity of care.Monitor allergies and prescribed diet constraints for accurate and appropriate implementation; ensure all clinical documentation reflects accurate and consistent information (including – but not limited to – HRST, onsite medical information “red” book).Know and routinely monitor individual parameters for vital signs, intake/elimination tracking, blood sugar monitoring, etc.; provide monitoring to ensure appropriate and accurate reporting, and adherence to individual protocols.Monitor the need lab work; collect data and ensure on time follow up if plete individual Health Risk Screening Tools (HRST): 1) at least once annually (between 90 and 120 days prior to the Individual Service Plan expiration date), 2) when a person experiences significant changes in health, functional or behavioral status, 3) when there is a change or discontinuation of medication, 4) upon all hospitalizations and/or emergency room visits, 5) when there are significant changes in behavior, and 6) when there are increases in the frequency of complaints from the person regarding how s/he is feeling or acting.Develop and maintain a working knowledge of relevant funding, licensing and accrediting body regulations and standards.Ensure individual rights are appropriately maintained; report violations.Participate in team meetings necessary to ensure individual health and wellness.Interact positively and effectively with individuals, families, external monitors/auditors, and all levels of UCP staff.Remain in active communication with supervisor on all program matters.Maintain objectivity in position in order to set appropriate limits while working with individuals.Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.Adhere to all agency policies and procedures and applicable program regulations.Other duties as assigned.SIGNATURE LINES: Sign below to indicate that the above statements have been reviewed with the employee by their immediate supervisor.Employee’s Signature _________________________________________________________ Date __________________Supervisor’s Signature _ Date ________________Rev. 9/30/15 AE ................
