|PROGRAM: |

|Internal Medicine Services at The Prince Charles Hospital comprises: |

|Geriatric, Rehabilitation and General Medicine Services. |

| |

|Geriatric services are provided across the continuum of care and include: Community geriatrics (Community Indigenous and Subacute |

|Services CISS) , Geriatric Outreach Assessment Service to nursing homes (GOAS), Older Persons Acute Assessment and Liaison (OPALS) in |

|the Emergency Department, Geriatric Evaluation and Management (GEM), Cognitive Assessment and Management (CAM) unit, Hip Fracture Unit |

|(provides orthogeriatric services in a unique shared-care model and provides a peri-operative medicine service to other surgical units).|

|The Geriatric and Rehabilitation Liaison Service provides geriatric consult liaison service and assesses all referrals for subacute |

|care. |

| |

|General Medicine, including an Early assessment Medical Unit (EMU) and acute general medicine unit) provides care for adult medical |

|patients utilising an age-friendly multi-disciplinary approach, with the support of a broad range of subspecialties. |

| |

|The Rehabilitation and Acute Stroke (RAS) unit led by rehabilitation physicians provides acute inpatient rehabilitation services, and |

|includes an acute stroke unit (co-managed by stroke physicians). Ambulatory rehabilitation services are provided by a Rehabilitation Day|

|Unit. |
