To our new and returning patients,

Thank you very much for choosing The Facial Surgery Center. On behalf of the entire practice, I would like to welcome you and assist you in any way that I can. Your pre-op package is enclosed. Please take the time to read through all of the material and familiarize yourself and your caregiver with the instructions. Your arrival time is included in this packet but due to possible adjustments in the schedule, the office will call you the day before to confirm your time.

Included in this packet, you will find:

A. Pre-op instructions

B. Medications/herbals to avoid preoperatively

C. A listing of commonly prescribed medications

D. Specific post-op instructions pertaining to your procedure(s)

E. Current Daily Medications List (please complete with current home medications/herbals/supplements)

F. Surgery Checklist (please review this list, initial each item, sign/date at the bottom before you arrive)

G. Prescriptions - to be filled

VERY IMPORTANT: Please bring all prescribed medications (filled) AND this information packet with you on the day of surgery.

Your actual prescriptions are the last items included in this packet (one or two pages). We do NOT stock medications here, so it’s VERY important that you bring ALL of them with you.

Also, please do not take any medications that we prescribe for you prior to your arrival. First, you will sign your consent(s), we will go over medications and instructions. Next, we will give you the necessary medications for surgery. If you take any heart or blood pressure related medications, take them like you normally would. If you are unsure whether or not to take other specific medications before surgery, please call the nurse.

Pre-Op Instructions

Please read the following additional guidelines carefully and call with any questions.

1) Do not take any supplemental vitamin E or aspirin-containing products for 1-2 weeks prior to surgery (although daily multi-vitamins are ok to take.) Both have been found to increase bleeding in surgical patients; take Tylenol for pain in the week prior to surgery.

2) On the day of your procedure, please take any heart or blood pressure related medications as you normally would. If you take medications for your blood sugar, check with the nurse. Anesthesia patients will not be eating before surgery, and taking blood sugar medication could lower blood sugar too much. Other medications may be re-started after surgery per our instructions- call with any questions.

3) Do NOT take any of the medications that we prescribe for you before surgery. Bring them ALL with you on the day of surgery and we will give you what you need.

4) For anesthesia sedation patients only - nothing to eat or drink after midnight on the evening of surgery. For all other patients (local anesthesia), be sure to eat prior to coming in.

5) Wash your face and hair as normal the night before or morning before surgery. Please do not wear any cosmetics, contact lenses or jewelry on the day of surgery. Wear a button or zip down top with comfortable pants and shoes (consider wearing or bringing socks because the OR tends to be on the cool side). Do not wear any pullover tops.

6) If you wear dentures, please bring a storage case with you. Depending on the procedure, we may ask you to remove them.

7) Do not color hair for 3-4 days prior to and at least 2 weeks following surgery, depending on surgeries done (this doesn’t apply to all procedures).

8) Avoid tanning beds and overexposure to the sun prior to and following surgery or laser procedures.

9) Please contact us immediately if you develop any signs of infection or illness such as fever, nausea/vomiting, cough or nasal drainage. Your surgery may be rescheduled to avoid any complications.

10) Patients who smoke must stop at least 3 weeks prior to and following facelifts. Smoking greatly compromises the body’s ability to heal and can have disastrous effects on your outcome. No exceptions will be made and patients who do not quit will have their surgeries rescheduled for a later date.

11) Arnica Forte (Arnica + Bromelain) is an optional herbal supplement that can be started the day of surgery AFTER your procedure. If it is not provided as part of your surgery quote (“healing supplements”) and you would like to take it, it is available for purchase through our office.

Herbal Supplements & Medications to Avoid

Review the following list and STOP taking any of the named medications for at 1-2 weeks prior to and following surgery. Many of these medications can cause increased bleeding and high blood pressure during and after surgery. Different supplements can also interact with other medications you may be taking for your procedure.

• Bilberry (Vaccinium myrtillus)- antioxidant; visual acuity- can increase bleeing

• Chondroitin- osteoarthritis- can increase bleeding

• Dong quai (Angelica sinensis)- relief of menopausal disorders, menstrual cramps

• Echinacea (Echinacea angustifolia)- immune-system stimulant- can interfere with healing

• Ephendra (Ma huang)- cns stimulant- can cause cardiac problems

• Feverfew (Tanacetum parthenium)- migraine preventative; allergy symptom relief

• Fish oil – contains omega-3 derivatives DHA & EPA; used to treat hypercholesterolemia and increased triglyceride levels

• Garlic (Allium sativum)- antispasmodic, antiseptic, antiviral, antihypertensive; used to treat hypercholesterolemia- can increase bleeding

• Ginger (Zingiber officinale)- antiemetic; antispasmodic- can increase bleeding

• Ginkgo (Ginko biloba)- antioxidant; enhances cerebral blood flow; alleviates vertigo and tinnitus- can increase bleeding

• Ginseng (Panax ginseng, P. quinquefolium)- improves physical and cognitive performance; antioxidant- can increase bleeding

• Glucosamine- osteoarthritis

• Goldenseal (Hydrastis canadensis)- mild laxative; reduces inflammation

• Hawthorne (Crataegus laevigata)- used for ischemic heart disease, hypertension, angina, and chronic congestive heart disease

• Kava kava (Piper methysticum)- sedative, analgesic, muscle relaxant, anxiolytic- can slow recovery and interfere with effects of anesthetics

• Licorice (Glycyrrhiza glabra)- used to treat gastric and duodenal ulcers, gastritis, and bronchitis- can interact with pain meds

• Melatonin- used for jet lag, insomnia, and seasonal affective disorders- can interfere with anesthetics

• Milk thistle- hepatoprotective, anti-inflammatory

• Red clover (Trifolium pretense)- used to relieve symptoms of menopause

• St. John’s wort (Hypericum perforatum)- antidepressant for mild to moderate depression- can interfere with pain medications

• Valerian (Valeriana officinalis)- sleep aid, mild sedative- affects pain medicines and can slow recovery

• Vitamin E- antioxidant; used in treatment of cardiovascular disease- can increase bleeding

• Yohimbe (Corynanthe yohimbe)- aphrodisiac, sexual stimulant- can interact with pain medicines

• Chamomile- can act as a blood thinner

• Cayenne- can act as a blood thinner and increase bleeding

Commonly Prescribed Medications

Below is a list of medications that could be prescribed to you for your procedure (we have marked the ones you have been prescribed). Please take all prescriptions (located at the end of this packet) to be filled and bring ALL of them with you the day of surgery. We will give you the medications to take after you sign your consent(s) on the day of surgery.

______ Diazepam (Valium) 5mg – (pre-operative) anti-anxiety. Used for local

anesthesia cases only. You will take prior to surgery in the office after signing consents.

______ Halcion 0.25mg - (pre-operative) anti-anxiety. Used for local anesthesia cases

only. You will take prior to surgery in the office after signing consents.

______ Cephalexin (Keflex) 500mg - antibiotic. Take 1 capsule three times a day for 3 days with food; take

all prescribed capsules.

______ Clindamycin (Cleocin) 300 mg -antibiotic. Used for penicillin allergies and cheek augmentation.

Take 1 capsule three times a day for 3 days with food; take all prescribed capsules.

______ Hydrocodone (Lortab) 5mg/325mg - pain relief. Take 1-2 pills by mouth every 4-6 hours as needed.

Take with food and use promethazine as needed to prevent nausea.

______ Oxycodone (Percocet) 5/325 mg - pain relief. Used for codeine allergies only. Take 1-2 pills by

mouth every 4-6 hours as needed. Take with food and use promethazine as needed to prevent nausea.

______ Odansetron (Zofran) 8mg - (pre-operative) anti-nausea. One to two tablets will be given before

surgery (at the office)

______ Promethazine (Phenergan) 25mg suppositories - (post-operative) anti-nausea. Take 1 suppository

rectally every 4-6 hours. If nausea already present, take Promethazine, wait 15 minutes, eat

something, then take pain medicine.

______ Medrol dosepack (methyl prednisolone) 4mg – Helps reduce post-operative swelling/inflammation.

On the day of surgery, we will instruct you on when to start taking this medication and you will simply follow the instructions on the actual pill pack.

______ Maxitrol Opthalmic Ointment – antibiotic ointment- Used for eye surgery only. Apply twice daily

to visible incision lines after cleaning. Do not apply to eye itself.

______ Chlorhexidine Gluconate 0.12% oral rinse (Peridex mouthwash) - anti-bacterial. Used for cheek implants only. Swish and spit am/pm and after meals.

______ Acyclovir 800mg- anti-viral. Used to prevent cold sore outbreaks in patients with a history of

outbreaks when surgery/laser around mouth. Take 1 pill 3 times a day, start day of surgery and continue until finished.

______ Arnica Forte with Bromelain (healing supplements) - Used to help reduce post-operative bruising

and swelling. Take 2 capsules daily with water, can start day of surgery after your procedure for

7 days. If you would like it and it is not listed on your surgery quote as “healing supplements”, you may purchase in the office on the day of surgery.

It is important that you fully understand the items listed below. If you do not adhere to these items, you may run the risk of your procedure being rescheduled. Please initial that you understand each of the applicable areas and sign below. Bring this completed form with you the day of surgery.

______3. You must have a responsible, competent adult bring you to your procedure, stay and listen to post-operative instructions, and take you home afterwards. Your caregiver does not need to wait in the office for the duration of your procedure, we will call them when it is time to pick you up. If applicable, someone may need to stay with you that night, and bring you to your appointment the following day; after certain surgeries you will not be able to drive. You also give us permission to discuss any post-operative care instructions with your driver. It is important that you make arrangements to spend the night in town after facelift surgery or otherwise as requested by Dr. Hochman.

______4. I have received and reviewed a copy of the post-operative instructions; I also agree to let my caregiver read these instructions.
